Objective: the main purpose of this experiment research is mainly aimed to discuss the clinical guiding significance on the cervical lesion screening by making use of thin prep cytology test(TCT), HPV somatotype detection together with human papilloma virus L1(HPVL1) shell protein detection and make comprehensive evaluation about the progressive risks of cervical lesion by predicting and analyzing TCT detection, HPV somatotype detection together with HPVL1 shell protein detection.Methods: 1: from 2014 June-2014 year in December the First Affiliated Hospital of Xi’an Jiaotong University School of medicine for gynecological examination gynecological clinic or found in cervical lesions to cervical exfoliated cell samples from 255 cases of type TCT and HPV-DNA detection at the same time, the detection results of at least one positive: HPV(+) or TCT(+) or two also tested positive for 95 cases of patients underwent colposcopy and biopsy. And the HPV genotyping for(+) and the expression of HPVL1 protein in the cervical tissues. And compared with histopathologic results.2. Examine the development of HPVL1 shell protein in different cervical tissues on the tissue section by using chemical way to immunologic tissue. Evaluate and analyze the matching rate between TCT, HPV and HPVL1 shell protein and the result of pathological tissue biopsy as well as their relationship.Result: Among inflammation, CINI, CINII, CINIII and SCC, the positive rate of HPVL1 shell protein is as follows: 65%(39/60), 57.1%(4/7), 14.8%(4/27), 0%(0/1) while the positive development rate of the HPVL1 shell protein for inflammation group and CINI is obviously higher that for CINII/III and SCC group, which shows that it has statistical significance. With the worsening of cervical lesion, the positive development rate of HPVL1 shell protein becomes lower and lower, while there is even no positive development for SCC.Conclusion: this research finding shows that: the high-risk infection rate may result from organism having no way to produce immune reaction so as to clear pathological cell because of the lack of HPVL1 shell protein development and therefore worsening the disease and finally leading to cervical cancer. Finally we can draw the conclusion: from the low-degree to middle-degree atypical lesion of squamous epithelium, cases of negative HPVL1 shell protein tends to progress more easily than those of positive HPVL1 shell protein. Therefore it can be used as a biological indicator to predict the progress of pre-cervical cancer. Examining HPVL1 shell protein has great significance in predicting early-phase of CIN lesion and in judging prognosis. The effective combination of examining the three indicators, evaluation and analysis has many clinical guiding values on the diagnosis of cervical lesion and the treatment.
