BackgroundSince Hepatitis C virus (HCV) was found by Americans in 1989, HCV infection rate increased year by year, which becomes a public health problem in worldwide. Due to the high variability of HCV’s genome sequence, hepatitis C virus is considered to be a kind of hepatitis virus which is most likely to cause mutation. According to the genetic difference of HCV gene, HCV can be divided into different genotypes and subtypes. We can use HCV genotype to make phylogenetic origin and spread of the virus explicitly. By understanding the geographical distribution of different genotypes and subtypes, we can make a further prediction about the antiviral treatment of interferon (IFN) on HCV. HCV-RNA can be used for early diagnosis of HCV infection, ALT and AST are two sensitive indicators who can reflect the damage in liver cell, therefore the monitoring of HCV genotype and HCV-RNA, ALT and AST has important clinical significance on the treatment of HCV.ObjectiveIn this study, plasma samples from 253 cases hepatitis C patients in Luoyang were used to detect these indices like genotype, viral load (HCV-RNA) and ALT, AST. Then indices were analyzed to investigate the relationship between the three, looking forward to provide reference for clinical treatment.Method1. Specimen Collection:Selected 253 cases HCV infected patients who came to Luoyang Central Hospital for treatment in 2012-2015, who hadn’t received antiretroviral therapy, and draw their venous blood on an empty stomach,3 ml blood in EDTA-K2 anticoagulant tubes,3 ml in sodium citrate coagulation promoting tubes. After centrifugal separation, the plasma was placed in the EP tube after high pressure sterilization, and then preserved in -70℃ for unified detection.2. Index detection:We took the detection of HCV-RNA, ALT, AST of all the collected samples, then for the samples that HCV-RNA>1×103 IU/ml, we deteceted the HCV genotype.3. Statistical Analysis:According to the results of the HCV-RNA, the samples were divided into four groups:A:minimumviral load, B:low viral load, C:medium viral load, D:high viral load, then analysing the distribution of HCV gene in Luoyang, and using SPSS 19.0 software package to analyse that whether the differences of ALT and AST lever are exist among A, B, C, D four groups, and whether the differences of HCV gene distribution are exist among B, C, D three groups.Results1. Five kinds of genotypes were detected in 253 cases of HCV infection in Luoyang area. They are 1b,2a,3a,3b and 6a respectively. Among them, lb accounts for 62.75%, followed by the 2a (30.39%), the rest of the three kinds are few. And there were 9 cases whose genotypes couldn’t been detected, maybe these 9 cases have mixed genotypes or other gene subtypes.2. The detection rate of genotype lb is the highest in the high viral load, and the detection rate of genotype 2a is the highest in the middle viral load, the difference has statistical significance.3. Among the A, B, C, D four groups, ALT, AST increased with the increase of HCV-RNA, the difference has statistical significance. Since there are many factors affecting the lever of ALT and AST, further research is needed.ConclusionHCV lb is the most predominant subtypes among hepatitis C patients in Luoyang area,followed by 2a, and the detection rate of genotype 1b is the highest in the high viral load,with 2a highest in the middle viral load.
