| Objective:Through the collection and analysis of clinical cases, we observe and assess the clinical effect of tonifying kidney, clearing away damp-heat and activating blood treatment on proteinuria in chronic glomerulonephritis.Method:The thesis adopts the retrospective observation research methods, and selects 61 patients from Nephropathy Department of Jiangsu Province Hospital of Traditional Chinese Medincine, whose follow-up information was complete and without loss, who were diagnosed with primary chronic glomerulonephritis. It includes 28 patients diagnosed with CKD stage 1, 19 patients diagnosed with CKD stage 2,14 patients diagnosed with CKD stage 3; 13 patients diagnosed with MsPGN,6 patients diagnosed with FSGS,2 patients diagnosed with MN,1 patient diagnosed with MPGN,1 patient diagnosed with MCD. By the treatment of tonifying kidney, clearing away damp-heat and activating blood for 1 year, before and after the treatment we make statistics and then analyse the security indicators and the effect indicators which include urine protein qualitative,24-hour urinary protein quantitation, urine latent blood, urinary red blood cells, Scr, BUN, Cys-C, eGFR.Results:After the treatment of tonifying kidney, clearing away damp-heat and activating blood, for 61 patients who were treated for 3 months,9 months,12 months, total effective rate is 70.5%,77.0% and 85.2% respectively. After the treatment, the therapeutic effect of urine protein quantitiative,24-hour urinary protein quantitation, urine latent blood, urinary red blood cells, Scr, Cys-C and eGFR has statistic significance(P<0.05). For patients diagnosed with CKD stage 1 and CKD stage 2, the therapeutic effect of urine protein quantitiative,24-hour urinary protein quantitation, urinary red blood cells and Scr has statistic significance(P<0.05). For patients diagnosed with CKD stage 3, the therapeutic effect of urine protein quantitiative doesn’t have statistic significance(P>0.05), but the therapeutic effect of 24-hour urinary protein quantitation, urinary red blood cells and Scr has statistic significance(P<0.05). Patients diagnosed with MsPGN who were treated for 3 months,9 months,12 months, total effective rate is 61.5%,76.9% and 92.3% respectively. Patients diagnosed with FSGS who were treated for 3 months,9 months,12 months, total effective rate is 66.7%,83.3% and 83.3% respectively. For patients diagnosed with MsPGN and FSGS, the therapeutic effect of 24-hour urinary protein quantitation has statistic significance(P<0.05).Conclusions:The treatment of tonifying kidney, clearing away damp-heat and activating blood can effectively control the level of urine protein, urine latent blood, urinary red blood cells, Scr, Cys-C, eGFR. Long-term treatment can consolidate curative effect, reduce proteinuria and have certain effect on protection of mildly and moderately impaired renal function, in order to delay the progress of renal damage. The treatment of tonifying kidney, clearing away damp-heat and activating blood can effectively control the level of urine protein, urine latent blood, urinary red blood cells, Scr for CKD stage 1～3, and long-term treatment can consolidate curative effect. The treatment of tonifying kidney, clearing away the damp-heat and activating blood is effective for MsPGN and FSGS. The treatment of tonifying kidney, clearing away damp-heat and activating blood doesn’t have obvious side effect, and it has good security. |
