[Objective] Allergic purpura is a common vascular allergic disease, its pathogenesis is mainly considered to be systemic vascular inflammation mediated immune factors. According to the clinical symptoms, it can be divided into simple skin type, joint type, abdominal type, kidney type and mixed type. Children and adolescents have a high incidence in clinic. Also, in spring and autumn, there is a higher prevalence. Its etiology is complicated and difficult to determine. The known ones include infection, drug and certain food, but others such as pollen, dust, vaccination, insect bites and cold stimulation are all likely to become allergens. Allergic purpura is characterized by recurrent attacks. Clinicians of western medicine treat it mainly by vitamin C,compound Lu Ding tablets and other drugs which can improve the vascular permeability, even glucocorticoid, cyclophosphamide and other immunosuppressants, which have apparent side effects, besides, there are still some patients refractory protracted, and can probably develop into the impairment of the renal function in the end. According to many modern physicians long-term clinical experiences, the common treatment of allergic purpura is combining the traditional chinese medicine with western medicine in the domestic, which has got a good effect. This study is based on the basic theory of traditional chinese medicine, under the guiding ideology of "Stagnant heat" theory and "Complex pathogenesis"theory, then we select the self-made prescription named Qing Dian Yin as a basic prescription, which results from the clinical experiences of a number of traditional chinese medicine physicians in the Department of Hematology belonging to the JiangSu Province Traditional Chinese Medicine Hospital. This clinical research is conducted during the 97 patients with allergic purpura, which can provide a scientific basis for the methods of heat hearing and detoxicating, cooling blood and removing blood stasis.[Method]Use the design method about a clinical observation rooting in the 2002 edition of the "the Guiding Principle of Clinical Research on New Drugs of Ttraditional Chinese Medicine" edited by Zheng Xiaoyu, and obey the test principle of contrast to make a clinical observasion and research. We bring these patients into the chinese medicine group, who are in accordance with the inclusion criteria and reject the treatment of glucocorticoid; others are included in the integrated chinese and western medicine group, who are taking glucocorticoid at the diagnosis and willing to continue to take it. According to the third edition of "Blood Disease Diagnosis and Efficacy Standards" edited by Zhang Zhinan and the 2002 edition of the "the Guiding Principle of Clinical Research on New Drugs of Traditional Chinese Medicine" edited by Zheng Xiaoyu, we can formulate the diagnostic criteria, inclusion criteria and exclusion criteria of allergic purpura. We deal the patients with general treatment and the adjunctive treatment of western medicine, at the same time, we treat them with the prescription of chinese medicine granule(hospitalized patients using traditional chinese medicine materical crude slices) based on the Qing Dian Yin for four weeks as a treatment cycle. We use the method of statistical integration to observe and analyse the changes before and after the treatment in the following three aspects:symptoms, urinary protein and urinary occult blood of the chinese medicine group and the integrated chinese and western medicine group after one treatment cycle. After the end of treatment, the patients are followed up for eight weeks to assess the clinical efficacy. Particularly, we continue to follow up the the well-healed patients for another eight weeks in order to evaluate the stability about the curative effect of chinese medicine in treating allergic purpura within eight weeks.[Result] 1.The chinese medicine group contains 76 cases, and the integrated chinese and western medicine group has 21 cases. There are significant differences about the symptom scores and the urinary test scores in the two groups before and after the treatment; 2.There are 48 cured cases, 24 improved cases and 4 ineffective cases in the chinese medicine group, and its total efficiency is 94.74%; there are 4 cured cases, 17 improved cases and 0 ineffective cases in the in the integrated chinese and western medicine group, and its total efficiency is 100%, and its total efficiency is 100%; 3.Treating the allergic purpura by modified Qing Dian Yin after 4 weeks, we can get the withdrawal rate of glucocorticoid is 100%; 4.Treating the allergic purpura by modified Qing Dian Yin, we can get the recurrence rate is 4.17% in the chinese medicine group, meanwhile, the the recurrence rate is 25% in the integrated chinese and western medicine group within 8 weeks.[Conclusion] 1. The modified Qing Dian Yin has an exact effect in treating allergic purpura, and it can effectively relieve patients with skin purpura, joint pain, abdominal pain and other symptoms, also, it can improve urinary occult blood and urinary protein markers in patients. The total efficiency is 94.74% in the chinese medicine group, and the total efficiency is 100% in the integrated chinese and western medicine group; 2. The modified Qing Dian Yin can shorten the use of glucocorticoid, and the withdrawal rate is 100% after 4 weeks,; 3.The modified Qing Dian Yin has a stable curative effect, and the recurrence rate is lower whithin 8 weeks.
