Background and Objective:Migraine is a complex neurological disorder characterized by severe pain, nausea, and exaggerated sensitivity to external stimuli such as light and sound.Migraine is one of the most common neurological disorders.In 20% of patients, transient neurological signs(the aura). It ranks among the world’s most disabling medical illnesses. The economic and societal effect of migraine is substantial: it affects patients’ quality of life and impairs work, social activities, and family life.The cause of migraine pain is controversial, with the most familiar theory proposing that this pain originates from inappropriate cerebral vascular dilatation. More recently, it has been proposed that migraine pain may originate from inflammation of the meninges,particularly the dural membranes that surround the brain. This theory proposes that, during a migraine, there is an idiopathic activation of trigeminal sensory afferents, resulting in nociceptive transmission to the CNS as well as the release of pro-inflammatory substances in the periphery.Dural inflammation is thought to lower the nociceptive threshold of dural afferents and facilitate nociceptive transmission to the central nervous system.Trigeminal sensory afferents are activated by electrically stimulating the trigeminal ganglion. This stimulation causes trigeminal peripheral sensory afferents to depolarize, inflammatory substances to be released from these afferents, and dural inflammation to appear.One of the consequences of trigeminal activation is the peripheral release of inflammatory mediators such as calcitonin gene-related peptide(CGRP) and substance P(SP) neuralpeptides. These peptides act in concert on the dural vasculature to cause changes characteristic of sterile, or neurogenic,inflammation.Trigeminal nerve vascular reflex theory is the Mainstream theories to the pathogenesis of migraine.There is evidence that many inflammatory cytokines are involved in the migraine, such as IL-1, IL-6 and IL-8, IL-17 and TNF-α. However, there is no correlation reports about CypA as proinflammatory cytokines involved in the occurrence of migraine.By binding to the cell membrane receptor CD147 they elicit a variety of intracellular signaling cascades involved in inflammatory processes. Increased levels of cyclophilins in inflammatory tissue and body fluids are considered to be a sign of an inflammatory response to injury.There is no correlation that CypA as proinflammatory cytokines involved in the occurrence of migraine.This study by examining patients with migraine episodes CypA level in serum and investigate whether CypA participate in migraine attacks.Whether CypA levels involved in migraine aura symptoms and the role of CypA in migraine pathophysiology mechanisms and provide theoretical basis for the prevention and treatment of migraine.Methods:This study collected 100 cases during March 2013 to December 2014 at the Liaoning Provincial People’s Hospital neurology outpatient and hospitalized patients. All the migraine diagnoses were based on the International Headache Society and were diagnosised by two neurologist.With the exception of exclusion criteria, meet the standards into group a total of 60 cases include 36 females,and 24 males.And according to the presence of aura is divided into two groups: migraine aura and migraine without aura group.40 cases healthy individuals for age, sex-matched patient group were selected, which are control group and include 25 females,and 15 males.This study has informed consent and record the basic information for each subjects.Using enzyme-linked immunoassay(ELISA) to detect CypA levels in serum.The analysis and processing of data use statistical analysis software SPSS 17.0, count data expressed as a percentage, measurement data are expressed as `x±S,meancomparison between the two groups using t test, P < 0.01 for the difference was statistically significant.Results :1.Seurm levels of CypA in migraine patients were significantly higher than seurm levels of CypA in control group, the difference has statistically significant(P < 0.01).2.There was no significant difference between migraine with aurag roup and level of CypA in seurm of migraine without aura group(p>0.05).Conclusion:1.The onset of migraine patients may be related to levels of CypA in seurm increased, CypA might be involved in the pathogenesis of migraine, detecting thelevels of CypA in seurm of migraine patients can provide a good reference for clinical diagnosis and treatment indicators.2. Seurm level of CypA may have nothing to do with migraine aura.
