| Object: Observe on clinical efficacy of knee osteoarthritis treated by different acupuncture therapies,we intent to find the best way to treat knee osteoarthritis.And investigate the change of mechanical pain threshold, heat pain threshold,ankle thickness,and IL-1βexpression in skin tissue of SD rats,which are treated by Electroacupuncture( EA) apparatus with acupoint injection function.Under this experiment, we aim to confirm analgesic effect and safety of the special apparatus, and provide a scientific evidence for clinical application. Methods: this part is divided into clinical study and animal experimental in two aspects.Clinical study: We selected the patients who were treated in acupuncture department of Wuhan Hospital of Integrated Chinese and Western Medicine from july July 2013 to July 2014 by machine and manual detection.These patients were divided into three groups,EA group,PI group and EA+PI group,according to different treatment therapies.And we classified their history medical data into 4 parts:1.general information:name,gender,age,etc.2.character of knee X ray,knee local condition and general condition. 3.treatment therapy:electro-acupunctrue,acupointinjection,electro-acupunctr ue with acupoint injection.4.efficacy evaluation.Animal experiment:Select SD inflammatory pain model rats,divided them into vehicle control group(blank group), inflammatory pain model group(control group),EA with acupoint injection group(combination group), EA apparatus with acupoint injection function group(special apparatus group).The combination group was administered acupoints injection in GB30(left) and GB34(left) with compound angelica injection by 1ml syringe after 2 days of modeling,then acupuncture these 2 points for 30 mins with special apparatus,which was setted frequency of 2Hz,pulse width of 0.3ms,the current intensity 1mA,3V voltage. Treatments were performed on the 2nd, the 4 th, the 6th day after modeling. The special apparatus’ s needles was thin and hollow,with which we administered compound angelica injection in this group,other intervenations of special apparatus group was similar as combination group.The modle group was only modelled and blank group was given non-intervention. Pain threshold behavioral response,threshold and ankle thickness were obeserved 1-6 days after modeling.After the last treatment(the 7th day), IL-1βexpression was detected in inflammation tissue by WB assay. Results:Clinical study:all the three therapies have effect on knee osteoarthritis,while EA combine with acupoint injection is more effective than other treatments,the difference between them is statistically significant(p<0.05).Animal experiment: Mechanical pain threshold, heat pain threshold of modeling rats were decreased, and foot dorsum swelling degree were increased after CFA injection. After treatment, EAPI group was more effective on increasing the mechanical pain threshold, heat pain threshold and reducing foot dorsum swelling degree than EA+PI group. Both of EAPI group and EA+PI group significantly reduced IL-1β expression induced by CFA. Conclusion:Clinical study: All of EA, acupoint injection, EA combine with acupoint injection have effect on knee osteoarthritis, and EA combine with acupoint injection is the most effective treatment.Animal experiment: EA apparatus with acupoint injection function is more effective than EA combine with acupoint injection treatment of knee osteoarthritis.Dual stimulation by EA and drug is the most important mechanism of the role of the apparatus, which can improve the import effect and precisely control the amount of Chinese and Western medicine,reducing drug side effects and adverse reactions on the body.The invention of this apparatus is significance for the development of new acupuncture,improve meridian theory and accelerate the pace of acupuncture internationalization,and it has important practical significance for alleviating. |
