Background:With the increasing incidence of Crohn's disease, the number of patients with anal fistula of Crohn's disease has increased significantly in recent years.Crohn's disease complicated with anal fistula and other perianal lesions in our country has a high proportion, and the occurrence of anal fistula is closely related to intestinal inflammation. With the recurrence of Crohn's disease, multiple surgical drainage may be necessary. At present, the commonly used analgesics and measures have many limitations for Crohn's disease patients, and the operation is inconvenient. The psychological burden of Crohn's disease patients is aggravated by one or more postoperative pain.It is necessary to explore appropriate analgesic schemes for Crohn's disease patients.Acupoint injection has been widely used in analgesia, and it is easy to operate. It can be innovatively applied to Crohn's disease after anal fistula surgery. Objective:To explore a safe and effective analgesic method for Crohn's disease patients with anal fistula and evaluate the effect of Acupoint Injection on analgesia about Crohn's disease patients with anal fistula. Methods:Thirty patients with Crohn's disease complicated with anal fistula were selected from March 2018 to February 2019 in the Anorectal Department of Jiangsu Hospital of Traditional Chinese Medicine. They were randomly divided into two groups by simple envelope method.Fifteen patients in each group were randomly divided into two groups. The experimental group was treated with"0.25%ropivacaine at Yaoshu point" immediately after operation. The control group did not take this measure. The NRS scores were compared at 12h?24h?48h?72h,that occurred at rest and during the first three dressing changes after operations, the NRS score of the first pain and peak pain in the course of disease, the time of the first pain after operation, the times of remedial analgesia in both groups, the situation of urinary retention after operation, and the safety evaluation,etc.Result:(1)The NRS scores of the first pain,12h,24h and three dressing changes in the experimental group was significantly lower than, that in the control group(P<0.05);(2)The time of pain was prolonged, the number of relief analgesia was significantly reduced,P<0.05 had statistical significance;(3)The NRS score at 48h,72h,urinary retention, safety assessment and the number of hospital days after operation. There was no significant difference between the two groups(P>0.05).Conclusion:Acupoint injection has certain clinical effect in analgesia of Crohn's disease after anal fistula operation.Immediate injection of ropivacaine at Yaoshu point after operation can safely and effectively prolong the postoperative analgesia time of Crohn's disease patients with anal fistula, reduce the peak pain, significantly reduce the pain within 24 hours after operation and during dressing change, and also reduce the application of opioids.
