| Background and objectiveWith the dvelopment of economy and increasing aging population, some problems such as unreasonable allocation of health resources and defective community health care are gradually becoming prominent. Our country is large in population, so the development of community-based health service is the key point to the quality of ordinary people’s health care. As an important part in grassroots medical career, family physician plays a vital role in improving community health care. However, the training of family physicians in our country is not systematic, resulting in a lack of clinical emergency knowledge and skills and an imbalance of clinical physicians. Therefore, strengthening the training of first aid knowledge and skills is urgent for improving health care and solving existing problems in the community.The objectives of this study are to investigate the current status and needs of those family physicians, explore exsiting first-aid training problems of family physicians in Chongqing and the optimal training courses for them, standerlize the traning levels and processes of community first-aid physicians and then put forward the reasonable suggestions and improvements.Main contentsMy research use the following methods:1) searching the literature to gain the basic theories and related concepts; 2) gathering expert opinions by the Delphi method and understanding the current situation, needs and practical results after improvements of family physicians in first-aid training; 3) statistical softwares include SPSS13.0, Excel, Graph Pad Prism, etc.Firstly, the background and some related terms were explained based on literature search. In the second part, through investigating the training status and demands of current family physicians in Chongqing Community, I grasped the understanding situation and related training demands of family physicians on first aid knowledge, individual skills and comprehensive emergency cases, which may provide practical evidences for the set-up of scientific family-physican first-aid training. In the third part, After stratified analyzing the training situation, I also put forward my thinking on the current training status. In the fourth part, I observed the training effects of clinical comprehensive first-aid training to family physicians under the practical level. Through questionnaire surveys, I compared the comprehensive first-aid training system with the traditional manner and found that the former one may link with practice more closely and improve the family physicians diagnosis levels as well as their independent thinking and solving problems ability, which indicated that comprehensive first-aid training system is an effective approach for family physicians’ training. Last but not least, in the fifth part, I put forward the improvement measures based on the previous investigations. I put forward my rationalization proposals in the following four factors: â‘ exploration of the exsiting problems of Chongqing family-physician first-aid training;â‘¡ standard managment of teacher training process to Chongqing family-physician first-aid training;â‘¢exploration of the suitable training courses for Chongqing family physicians;â‘£improvment the first-aid skill levels of Chongqing family physicians. My research may provide practical and reference basis for the future family-physician first-aid training method in Chongqing.Main results1. Of the 210 family physicians in survey, the education levels were mainly undergraduate or below, physicians with high education levels are insufficient. The ages, gender ratios, education and majors of these physicians did not have significant difference. 70.5% family physicians had the experience of participating continued medical training and 53.4% of them had joined specific first-aid training, mainly were short terms. While 29.5% of these physicians did not have any medical training due to other reasons. The main methods for obtainin knowledge were teaching by other teachers. Therefore, it is urgent to further strength the continued medical education, especially the first-aid training for family phycians.2. Of the surveyed physicians, we mainly investigated the knowledge and skills of clinical common diseases, first-aid skills and comprehensive first-aid cases. The physicians who were quite skilled in these three points are 63%~85%ã€43%~96%ã€73%~83%, respectively, while the physicians who demanding them were as high as 84% ~94%ã€84% ~ 96% 〠81% ~ 90%, respectively. As a result, although the majority of family physicians could master the listed first-aid knowledge, specific first-aid skill and comprehensive skills, they had more desire for the further training of these contents, suggesting the necessity and importance of conductin g the first-aid training in community family physician.3. Searching for the main problems in community first-aid training by data analysis and literature searching. The main problems were as follows: insufficient of the training materials of first-aid training, defective in the courses and effective methods for these training and inconsistency in the assessing standards of first-aid trainings. After analyzing the problems of methods and contents in the clinical first-aid training in Chongqing, we explored the reasonable suggetions for these first-aid trainings.4. In the section of exploring the first-aid training in community family physicians, it was demonstrated that physicians did not have significant differences in age, gender, education and title. Compared with control group, the experiental group had more points in the field of practical work, diagnosing standards, solving problems, improving study motivation and participation, clinical thinking ability, teamwork spirits and cultivation of humanistic quality(P<0.01), while there was no signicant difference in the extent of standardization, targeted objectives and serious attitude.5. On the basis of learning the current situation and demands of first-aid training for family physicians in Chongqing, I put forward my thought about the training. By studying the practical methods in first-aid training, I mainly gave suggestions in the field of policy support, contents in training, teaching facilities, trying to improve the emergency knowledge and skills of family physicians in Chongqing.Main conclusionsBy investigation on the 210 family physicians, it was found that these physicians had strong desire for first-aid training. By effective training, the levels of mastering the common clinical skills and knowledge improved greatly. Therefore, establishing sophisticated managing methods, teaching contents and assessing standards for the first-aid training and exerting the power of supervising measures, are important in the cultivation of first-aid skills of community family physicians. Exploration of the process and standards of the clinical first-aid training and establishment of the first-aid training and community practice are vital for the improvement of first-aid skills and medical healthcare in Chongqing. |
