Clinical Intervention Effect Of Sequential Ttherapy Of Nourishing Yin And Tonifying Yang On Polycystic Ovarian Syndrome During Stimulation Cycles

Objective:To observe and analyze the clinical effect of sequential therapy of nourishing yin and tonifying yang in stimulation cycles among PCOS patients, to explore the functional mechanism of TCM. The study aimed to fine an effective and safety way for PCOS patients to get pregnant by using integrated traditional Chinese and Western medicine.Methods:79 PCOS patients in accordance with the inclusion criteria were enrolled and divided into two groups randomly:the control group(n=33) and the experimental group(n=46). Patients in control group were given Western Medicine (CC+HMG+HCG) for ovulation induction therapy for 52 cycles,while patients in experimental group were given traditional Chinese medicine for nourishing yin and Yang sequential combined with western medicine for 53 cycles. The two groups were compared in patients with follicular development and ovulation rate, endometrial thickness and morphology on HCG day, Gn duration, and Gn dosage, the pregnancy rate per cycle and final pregnancy rate, OHSS tendency.Results:There were 31 ovulation cycles, of which 6 cycles got clinical pregnancy in the control group,while 44 ovulation cycles, of which 15 cycles got clinical pregnancy in the experimental group. Clinical cure rate of two groups of patients (cycle pregnancy rate) and the total effective rate (cycle ovulation rate) were 11.54%,59.62% and 28.30% and 83.02%, respectively,both of the differences were statistically significant (P<0.05).The pregnancy rate was 18.18% in control group, while the experimental group 32.61%, of no significant difference (P>0.05).The duration of Gn was 8.21±4.38 days, significantly less than the control group of 11.31±4.78 days(P<0.05);the total Gn dosage in the two groups were 330.52±164.27IU and 567.55±390.28IU,respectively,which was of significant difference (P<0.05).The endometrial thickness in the control group was 7.32±1.47 (mm), in which 25 cycles showed type A.The endometrial thickness in the experimental group was 8.84±1.60 (mm), in which 42 cycles showed type A.Both the endometrial thickness and the eodometrial type showed statistical significance (P<0.05).The control group had 9 LUFS cycles, the ovulation rate was 75.61%; the experimental group has 2 LUFS cycles, ovulation rate was 91.67%, the difference was statistically significant(P<0.05). The rate of cycle cancelling showed statistical significance (P<0.05),and cancel test group of cycle number is less than the control group. The number of mature follicles、no follicle growth cycles、single follicle growth cycle、development of multiple follicles cycle、small follicular ovulation cycles、OHSS tendency cycles on HCG day were not significantly different (P>0.05).Conclusions:Nourishing yin and tonifying yang sequential traditional Chinese medicine combined with CC+HMG+HCG protocol in the treatment of patients with PCOS can improve the rate of cycle ovulation and pregnancy rate, increase endometrial thickness, improve the endometrial morphology, reduce the duration and dosage of Gn,reduced the incidence of LUFS, reduce the cycle cancellation rate.Nourishing yin and tonifying yang sequential traditional Chinese medicine combined with CC+HMG+HCG protocol is an effective assisted reproduction way for patients with PCOS.
