Clinical Study On Hyperandrogenism In PCOS Patients By Yin-Nourishing And Yang-Tonifying Law Sequential With Compound Cyproterone Acetate

Objective: To observe efficacy of Yin-nourishing and Yang-tonifying law sequential (YYFS) and compound cyproterone acetate on hyperandrogenism in PCOS patients and explore the mechanism.Method: Theory study and clinical observation were involved in this study. Selecting and analyzing classic and modern articles were the main methods of theory study. In clinical observation, 60 patients which were consistent with our study standard randomly were divided into two groups: group A (30 cases)treated with YYFS; group B(30 cases) treated with YYFS and Diane-35 for three months. The levels in serum of T,LH,FSH,E₂,SHBG,DHEAS were assayed. At the same time, the information, such as pattern of their menstruation, fatness, acne, hairness and so on, was collected from pretherapy.The therapeutic effects were evaluated through comparing the difference between before and after treatment. Also in the study,we explore the relationship beween kidney-yin,kidney-yang deficiency syndrome and T,LH of hyperandrogenism in PCOS patients.Results: The overall efficiency is 92.59% in group B and the same is 69.23%in group A , there is significant difference(P<0.05).The results showed that T,LH,LH/FSH,DHEAS levels in group B after treatment were significantly lower than before and SHBG levels in group B was significantly higher than before and its effeciency is prior to group A(P<0.05). YYFS and compound cyproterone acetate can modify the menstruation cycles and kidney-deficiency symptoms efficiently. After treatment,fatness,acne, hairness were all improved, and the effeciency is prior to group A. Through the TCM syndromes research, we find the patients with kidney-yin deficiency syndrome is majority. The serum T of the patients with kidney-yin deficiency syndrome is higher than the patients with kidney-yang deficiency syndrome(P<0.05). The serum LH of the patients with kidney-yin deficiency syndrome is higher than the patients with kidney-yang deficiency syndrome(P<0.05)Conclution: A negative correlation is found between the serum of SHBG and DHEAS on hyperandrogenism in PCOS patients . DHEAS which is excreted by the adrenal gland is concerned with the generate of PCOS.The hyperinsulinmia of the blood could induce a decrease of SHBG in the volume of circulation bloood, which could cause the imbalance of gonadal hormre ,and it is the reason that why the patient maifest the hyperandrogenism . The clinical efficacy of YYFS and compound cyproterone acetate on hyperandrogenism in PCOS patients is proved in the study. By amendmenting the function of hypothalamus-hypophysis- ovary axle, it can efficiencyly adjust the endocrine disorder conditions, degrade the serum of T,LH,DHEAS, upgrade the serum of SHBG, tonify menstruation, improve the clinical symptom. Integrative traditional Chinese medicine with western medicine is quite a good therapy to PCOS patients with hyperandrogenism.
