An Analysis On The Accuracy In Abnoamal TCT Results

Objectives:To evaluate the efficency of TCT in cervical cancer screening, the accuracy was analyzed by comparing abnormal TCT results with colposcopic biopsy results.Methods:58,718TCT cases were reviewed during December2013and November2014in Women’s Hospital, School of Medicine, Zhejiang University, among which, data from1633TCT positive cases who received further biopsy was systematically collected and retrospectively analysed.Results:The age of the patients in58,718TCT cases was normally distributed, covering16to92with an average of39.75±10.778. TCT positive ratio was found to increase by aging of the patient. The age of1633TCT positive patients were as well normally distributed ranging from17to89, with an average of41.61±10.547. A coincidence rate of83.3%was found between abnormal TCT and biopsy results (Kappa value at0.551), in which SCC got100%while LSIL got93.8%. The impact of age on the accuracy was concluded not significant. In relatively low-grade cytology especially ASC-US cases, the inadequacy of diagnosis was significantly lower in HPV negative than that of HPV positive (P<0.05).Conclusions:1. A high coincidence rate was found between abnormal cytology and biopsy, especially in HSIL and SCC, suggesting that patients with high-grade cytology should be directly examined by colposcopy.2. The accuracy of abnormal cytology was not influenced by age, indicating that it is especially suitable for those younger than30years old for whom HPV test was not recommended.3. In low-grade cytology, the inadequacy of diagnosis was significantly lower in HPV negative than that of HPV positive, suggesting HPV test was valuable for triage.
