Objective: Through the analysis of TCT and HPV combined screening of cervical lesions,the diagnostic value of high risk HPV in cervical lesions was discussed.Methods: To collect the basic information of the first diagnosed patients in the colposcopy outpatient clinic of the Yiji Mountain Hospital of Southern Anhui Medical College from August 2015 to August 2017.678 patients with clinical symptoms(leucorrhea,vaginal bleeding,contact bleeding)or any of the combined screening of TCT and HPV who needed colposcopy to perform cervical biopsy or Leap knife surgery were selected.Age 16-69 years,average age 39.4 years.Finally,the value of high-risk HPV in the diagnosis of cervical lesions was analyzed according to the standard of cervical histopathology.Results: Of the 678 specimens,293 were inflammatory lesions and 385 were cervical lesions.There were 87 cases of pathological inflammation and 37 cases of cervical lesions in the TCT(-)HPV(-)group.There were 138 cases of pathological inflammation and 193 cases of cervical lesions in TCT(-)HPV(+)group.There were 21 cases of pathological inflammation and 33 cases of cervical lesions in TCT(+)HPV(-)group.There were 47 cases of pathological inflammation and 122 cases of cervical lesions inTCT(+)HPV(+)group.According to the histopathological results,it was found that the combination of TCT and HPV-DNA detection had the highest sensitivity.The sensitivity of HPV detection alone was much higher than that of TCT alone.The missed diagnosis rate of TCT detection alone wasmuch higher than that of HPV alone.In TCT(-)hr-HPV(+)group,the infection rate of high risk HPV was 51.12%?85.47%?92.30%,respectively.With the upgrade of pathological results,the infection rate of high-risk HPV is on the rise.Regardless of the TCT results,the risk of cervical lesions in high-risk HPV infection patients is relatively high.Among all the subtypes of HPV infection,the infection rate of high-risk HPV subtype is 88.09%(732/831),and the infection rate of other HPV subtypes is11.91%(99/831).The CIN ??CIN? / ? ?CC was 79 ?170 ?19 in 300 patients with TCT(-)hr-HPV(+)group.In CIN patients,the first five HPV genotypes of the common high-risk type were the first five from high to low: HPVl6? 58?33?52?18.Conclusion:1.Patients with TCT negative and high risk HPV positive should take active measures to prevent missed diagnosis from developing into serious lesions.2.High risk HPV infection is an important factor in cervical lesions.Regardless of the results of TCT detection,the probability of cervical lesions in patients with high risk HPV infection is relatively high,and the sensitivity of high risk HPV infection is higher than that of single TCT detection.
