Relationship Among Resilience, Social Support And Quality Of Life In Patients With Depression

Objective:The study aimed to investigate the status of quality of life (QOL) in patients with depression, to analyze the relationship among resilience, social support and QOL. The study was performed to explore the level of resilience which was acted as a mediating role between social support and QOL so as to find methods to improve and enhance QOL in patients with depression from the perspective of positive psychology, which provide theoretical basis.Methods:The study included74patients diagnosed with depression that were taken from a mental health center in Shandong Province by adopting convenience sampling methods, and51cases were taken from the relatives and the medical staff in hospital as the healthy control group. In terms of general demographic information, there were no significant differences between the two groups. The two groups were assessed using Connor-Davidson Resilience Scale (CD-RISC-10), perceived social support scale (PSSS) and the quality of life questionnaire (WHOQOL-BREF) to measure the level of resilience, social support and QoL. The data were analyzed with SPSS17.0for windows and the methods of statistic analysis were applied including descriptive statistics, ANOVA, t test, correlation analysis and hierarchical regression analysis.Results:1. QOL in patients with depression was much lower than that of healthy individuals. The results of t test showed that the scores of self-assessment QOL and the subscale of physical, psychological, social and environmental field were lower than those of healthy control group (P<0.05-0.001). The methods of t test or One Way ANOVA accounted for the effects of different population sociology materials on QOL, The results showed that different degree levels of depressive patients have different self-assessment QOL, which has statistically significance. Different ages of individual in social field dimension have different scores (P<0.05). In addition, other dimension scored in different population sociology have no statistics differences (P>0.05).2. The levels of resilience and social support in patients with depression were lower than those of healthy control group. The results of t test showed that the levels of resilience in patients with depression had significantly lower scores than the control group (P<0.001).Compared to the healthy control group, the patients with depression had also significantly lower scores on the scale of family and friends support (P<0.05).There were no significant statistical differences on social support and other support dimension between the two groups (P>0.05).3. Correlation analysis revealed that there were significant positive correlations among resilience, social support and QOL in patients with depression, showing that the self-assessment scores of resilience and QOL in patients with depression have more moderate positive correlation (r=0.504~0.763, P<0.01). There were moderate positive correlations among the self-assessment scores of QOL, social support and other dimension respectively(r=0.401~0.536, P<0.01). In addition, there were positive correlations between the level of resilience and social support (r=0.433, P<0.01).4. Hierarchical regression analysis was used to explore the resilience in the intermediary relation between social support and QOL. The study applied each dimension of QOL as the dependent variables, social support as independent variables, general demographic variables as first-level control variables, social support as the second-level variable into the equation, and the level of resilience as third-level variables into equation. The results indicated:When used the self-assessment QOL, the scale of physiological and psychological field as dependent variables respectively, social support has significant predictive effect on these dependent variables (β=0.353,0.397,0.413, P<0.01~0.001). R2accounted for60.6%,54.1%,65.7%respectively, net variance accounted for18.9%,24.9%,41.9%respectively. Meanwhile, the main effects of social support on the dependent variable were no longer significant because of the introduction of resilience variables, which showed that resilience played a mediating role entirely in the relationship between the self-assessment QOL and the subscale of physical and psychological field.When used the subscale of social and environmental field as the dependent variable respectively, social support had significant predictive effect on both dimensions (p=0.483,0.376, P<0.01~0.001). R2accounted for54.1%、45.5%respectively, net variance accounted for12.9%、8%respectively. Meanwhile, the main effects of social support on social and environmental fields were reduced because of the introduction of resilience variable, which indicated that the level of resilience played a mediating role partly in the relationship between social support and the subscale of social and environmental fields.Conclusion:1. Because the presence of depressive symptoms induced low mood, patients with depression have poor QOL.2. The levels of resilience and social support in patients with depression are lower than those of healthy people.3. There is a positive correlation among resilience, social support and QOL in patients with depression.4. Both resilience and social support in patients with depression predict QOL significantly. Furthermore, resilience entirely or partially mediates the relationship between social support and QOL. Taking measures to enhance the level of resilience and increase the application of social support can improve QOL.
