Impact Of Job Stressors And Psychological Capital On Career Development Of ICU Clinical Nurse Specialists

Objectives This study was aimed at investigating job stressors, psychological capital and career situations of ICU clinical nurse specialists, exploring their relationships and the major influence factor of ICU clinical nurse specialists’ career status, and providing reference and basis for their career education and management.Methods Using Stratified random sampling methods, participants of this study were sampled from12tertiary-level hospitals in Shandong province. A total of247ICU clinical nurse specialists were investigated by questionnaires on job stressors, psychological capital and career conditon. SPSS Statistics21.0and Amos21.0were used to date entry and analysis.Results1. The job stressors score of Critical Care Nurses was92.13±20.17, average score2.63±0.58. Nursing profession and work problem, patient care problem and management and interpersonal scores were respectively (19.48±4.58),(29.38±7.15),(21.31±7.18), and average scores were (2.78±0.65),(2.37±0.80),(2.67±0.65) respectively. The median, interquartile range of time allocation and workload were3,0.6respectively. The median, interquartile range of work environment and resource problem were2.33,0.67respectively. Thus, the overall pressure source of ICU clinical nurse specialists was in the middle level.2. The psychological capital score was105.47±13.35, average score was (4.40±0.56). Each demension score:toughness score26.06±3.68(average score4.34±0.61), optimism score25.14±3.62(average score4.19±0.60). The median and interquartile range of self efficacy were4.67,0.34respectively. The median, interquartile range of hope were4.50,0.67respectively.3. The career condition score was79.19±11.63. Professional identity, job satisfaction, organizational commitment dimension scores were respectively (3.66±0.63),(3.23±0.804) and (3.28±0.52). The median, interquartile ranges of intention to stability were5and1respectively.4. There were statistical differences of career condition score in different characteristics of social demography. The career condition score of ICU clinical nurse specialists who were more than40-year-old was higher than those who were31-40. The career identity score of Critical Care Nurses who were more than40-year-old was higher than those who were20-40. The dimension scores of male nurse’s organizational commitment are higher than that of female. The career identity score, organizational commitment, occupation satisfaction of whose ICU working life was more than16-year-old were higher than those whose ICU working life were within5years,6-10years and11-15years respectively. The career condition score of ICU clinical nurse specialists whose critical care specialist qualifications obtained time7-9years of nurse was higher than thouse get it within3years. The career condition score, career identity score, occupation satisfaction score of those who participated in the occupation career were higher than thoes who have not.5. The psychological capital score and each demension score was positively correlated to the score of career condition score, professional identity, job satisfaction, organizational commitment. The career condition score was negatively correlated to the scores of job stressors score, nursing profession and work problem, work environment and resource problem, management and interpersonal problem.The work environment and resource problem score were negatively correlated to career identity. The career condition score score and each demension score were negatively correlated to career satisfaction.The time allocation and workload problem were negatively correlated to organizational commitment. 6. Multiple linear stepwise regression analysis for career condition score showed that, psychological capital score, time allocation and workload and qualifications obtained time were predictor variables of career condition for critically ill nurse specialism; all of these could explain31.8%of the total variances. Psychological capital was the main influencing factors for career condition.7. The path analysis results of realationship among nurses’ psychological capital and work pressure source and occupation career status showd that the psychological capital was an intervening variable for work pressure source and occupation career status. It means that most of pressure source of ICU clinical nurse specialists affected their ccupation career through psychological capital. There was a negatively correlated realationship between psychological capital and occupation career. The more psychological care are offered, the better their occupation career will be.Conclusions1. The scores of job stressors were in a medium level, nursing profession and work problem was the major source of stress.2. The scores of psychological capital and the dimensions were in a medium or higher level.3. The career condition score of Critical Care Nurses was in a medium level.The psychological capital score and each demension score was positively correlated to the score of career condition score;The job stressors score and several dimensions were negatively correlated to the scores of career condition score.Psychological capital score dimensions of time allocation and workload issues and achieved specialist qualification period were predictive variables for occupation career specialist nurses in critically ill patients.The psychological capital is the intervening variable between work pressure and occupation career, and it is a protect factor for nurse’s occupation career.
