Retrospective Study On Medical Records Of Children With Cerebral Palsy Treated By Jiannaoyizhisan

ObjectiveThrough to analysis patients’ medical records that Henan ruzhou Jingeng rehabilitation hospital’s traditional Chinese medicine comprehensive treatment on children with infantile cerebral palsy,preliminary evaluate its clinical therapeutic effect and safety.Which will help further determine and regulate the treatment plan, and thus provide useful guidance for clinical treatment and provide the foundation for further prospective studies.MethodsThis study is based on the hospital actual clinical data,adopts the design method of retrospective study,Set the standard of required medical records,make rules that the hospital medical record data be analyzable, extracting information and the effective index,beginning statistical analysis after double entry and data check.Medical screening methods:include all the medical records since January 2009 to April 2013 in hospital,based on all the qualified ones which meet the standard, with 2 people as a set to analysis the records, total 98 records were collected;Data analysis methods:Due to the patients are from the welfare home and social,basically the same treatment,but some evaluation indexes are different,period of treatment were also different.So, two people are divided into welfare home and society when analysis the curative effect. Some medical records were in process of hospital treatment,and period of treatment didn’t reach the requirements and so not be included.ResultsThe treatment period of the 48 records from welfare home all achieved 15 months.The treatment period of 50 records from social all achieved 6 months.1.For 48 cases of children from the welfare home on admission,respectively compared the ability of recumbent position and to roll over,sitting position,climb and knee seat,the three-dimensional, walking and runningof in hospital and after treatment of 3 months,6 months,9 months,12 months,15 month.P value is less than 0.001, there are significant differences.For 48 cases of children from welfare home,with 24 months for the world is divided into two groups,hierarchical analysis, found that after treatment under the age of 24 months total gross motor function scale increased by 63.65 points, more than 24 months years old after treatment scale total increased by 45.97 points, the younger better recovery, better treatment effect.2.For 50 cases from the social,evaluate the 50 indicators of daily life ability before and after the treatment for 3 months and 6 months,all the effects are good,6 months after treatment, on average, increased by 9.96 points.For 50 cases of children from society,with 48 months for the world is divided into two groups,hierarchical analysis, for the median number,found that younger than 48 months of treatment in children with 18.0 points, three months after treatment increased to 23.5 points,6 months after treatment increased to 27.75 points, many children raised 8.7 points; Older than 48 months of children, on figures, before treatment in children with 37.00 points, three months after treatment increased to 40.0 points,6 months after treatment up to 48 points, most of the children raised 11.21 points.For 50 cases from the social,with intelligence test report before and after the treatment curative effect assessment,with 48 months for the world is divided into two groups,hierarchical analysis,for the median numberless than 48 months of treatment in children with 7.00 points, before 3 months after treatment increased to 12.0 points,6 months after treatment increased to 21 points.Average increased by 9.93 points before treatment to 17.82 points after 6 months treatment, the average increased 8.89 points.Older than 48 months of children, for the median number, before treatment in children with 32.00 points, three months after treatment increased to 38.0 points,6 months after treatment rise to 39 points, many children raised seven points; Average increased by 28.43 points before treatment to 39.00 6 months after treatment, the average increased 10.57 points.3.Treatment options of give priority to traditional Chinese medicine JianNaoYiZhiSan(l)Composition:Tortoise shell、centipede、scorpion、ginseng、Chicken’s gizzard-membrane、Sea dragon、the hippocampus、Chuanxiong. Porphyrization and oral.lg one time for under the age of 1,2 g one time for 1 to 3 years old,3 g one time for 3 to 6 years old,2 times a day; Dosage was according to the weight of children,and 3 months for a course of treatment.(2)Discretionary household acupuncture、massage、PT treatment Acupuncture:best, four god astute, movement, language, sensory area; Massage: relieve muscle, improve spasm. "type A botulinum toxin injections" muscles.4.This study summarizes a set of method that converting the text of information into statistical analysis of data, including eight steps:determination of the objective, determination of the index, index definition and terminology standardization, coding and assignment, database design, data entry, quality control, database locking eight steps, each step is very specific research and operation process, this process can provide methodology for retrospective clinical study in terms of reference.ConclusionThis study systematically analyzed the medical records based on 98 patients who are diagnosed as cerebral palsy. With compound traditional Chinese medicine is given priority, with acupuncture, physical therapy treatment when necessary.The result show that:1.The treatment has a significant effect:after treatment with general conditions, sports ability, daily life skills, intelligence, language ability all have a greater degree of improve; As the extension of treatment time, the indicators showed a trend of sustained recovery and effect shows that the longer the treatment time, the better effectiveness is;The earlier the treatment is carried on, the better the recover is,illustrates the rehabilitation therapy treatment for infants and young children is very effective, and the smaller the age, the better effect is.2.The treatment is safe with no obvious side effects:Taking method of JianNaoYiZhiSan is given to children with food and is applicable for most of children, but individual children appear a "irritated" performance, such as bleeding, ulcers,which can be eased after symptomatic modulation; Not found damage in liver and kidney due to serious adverse reactions of drug;3.Retrospectively studied based on the hospital medical records,the key is converting the text of information into statistical analysis of data.The characteristic of the study is to make full use of the existing hospital information, using retrospective study method to carry on the scientific design and scheme expert argumentation, with the support of local hospital,after a rigorous screening、the medical records converting to an analysis index,careful check、system of statistical analysis, obtained the safe and effective research conclusion,Above conclusion is limited to retrospective study, a prospective study in the future is remained to verify the curative effect.
