In recent years, the incidence of cerebrovascular disease increasingly higher, and patients who had the tracheotomy after stroke because of their deliriums and central respiratory dysfunction also accounted for a certain proportion. Because of the indwelling tracheostomy tube, there comes the resistance of the rehabilitation process of the stroke, or even cause severe respiratory disease, and then life-threatening. Therefore, to explore treatments for patients with tracheotomy after stroke to give impetus to their rehabilitation of respiratory function is a problem in clinical work.Objective:To investigate the impact of acupuncture back-shu on respiratory rehabilitation of patients had tracheotomy respiratory after stoke on the basis of the general treatment and to open new ideas for them.Methods:We observed the curative effect of 50 cases of post-stroke patients with tracheotomy with a prospective single-blind randomized controlled clinical trial, who were admitted to the acupuncture and rehabilitation second of Guangdong No.2 TCM Hospital of form March 2013 to February 2015 for treatment, and were eligible for inclusion and exclusion criteria. According to the time of admission, these patients were numbered from 1-50, and then randomly divided into experimental and control groups according to the random numbers generated from the statistical software, and the latter were given the body acupuncture treatment in a general basis when the former added the acupuncture of back-shu points in the basis of the latter. The choice of back-shu points were BL13, BL2O and BL23. We treated once per day, and suspended one day every six days, till we treated for 4 weeks. According to " clinical research guidelines of Chinese new drug " we finished the respiratory symptom score and the respiratory symptoms efficacy classification of both groups before and after treatment, respectively, we calculated the total efficiency and total scores of the two groups, the chi-square test and t test were used for statistical assessment to estimate their efficacy before and after treatment; We compositely scored the eyes reactions, motor response, brain stem reflex and respiratory function of two groups of patients before and after treatment in accordance with FOUR Scale, and analyzed the score differences of each group before and after treatment, also the score differences between the experimental group and the control group after treatment with the t test of statistical analysis; statistical treatment of Noted the number of cases with successful extubation, failure extubation and delayed extubation, then calculated the success rate of extubation of two groups during treatment and analyzed with the chi-square test.Results:After treatment, the respiratory symptoms classification quantitative score and total score, FOUR score and extubation success rates during treatment of experimental group and the control group were obvious different, and there were statistically significant differences. The total effective rate of the test group was 90% and the total effective rate was 60% of the control group, between which the difference was statistically significant (P<0.05);Obvious difference existed after treatment about the FOUR scores and total score of respiratory symptoms of both the experimental and control groups, and the differences were statistically significance (P<0.05), the test group FOUR score after treatment with the control group, the difference was statistically significant (P<0.05); The extubation success rate of the test group was 64% and the extubation success rate of the control group was 32%, and the difference was statistically significance (P<0.05).Conclusions:Acupuncture on back-shu has the exact effect on the rehabilitation of respiratory function of patients have tracheotomy after stroke, which can improve their respiratory symptoms, and the total efficiency comes up to 90%, while promoting the rehabilitation of patients have tracheotomy after stroke integrally, including their eyes reaction, comprehensive rehabilitation motor response, brainstem reflexes and respiratory function, making synergies between pulmonary rehabilitation and integral rehabilitation,and also improve their chances of successful extubation. Thereby it promotes the rehabilitation process of post-stroke patients have tracheotomy integrally; and acupuncture on back-shu points are relatively safe, no side effects while this treatment method is simple, so it should be extended to the treatment of stroke patients with tracheotomy for their breathing dysfunction in clinical practice.
