Objecti veTo observe the clinical effect of " triple energizer acting on gas”acupuncture method combined with breathing training on respiratory function of patients with tracheotomy after stroke.At the same time,according to the correlation between diaphragmatic muscle function and lung function,M-mode ultrasound is used to evaluate the effect of this treatment method on the respiratory function of the patient more objectively and quantitatively,aiming to provide new ideas for the rehabilitation of respiratory function in patients with tracheotomy after stroke.Methods72 patients who were hospitalized in the Acupuncture and Rehabilitation Department of our hospital(Guangdong Second Hospital)from January 2018 to January 2019 were divided into observation group and control group randomly,36 patients in each group.Routine care(timed roll back,sucking,etc.)and basic treatment(conventional medicine,limb function rehabilitation training).The control group was given respiratory function training on the basis of routine nursing and basic treatment;the observation group was treated with triple-focus gas-adjusting acupuncture on the basis of the control group.The two groups were treated once a day,and it lasted six days 6 days and rest 1 day during a total of 28 days.Before treatment and 28 days after treatment,VTi,VTe,and MV were recorded by ResMed EliseeTM 350 respirator.Blood gas analysis was collected,partial pressure of carbon dioxide(PaCO2)was recorded and oxygenation index(PaO2/FiO2)was calculated.The US SonoSite portable color ultrasound M-Turbo was used to record the parameters of the diaphragmatic mobility of the patient;the respiratory symptoms before and after treatment were compared between the two groups.The scores were scored and then graded according to the scores.The total effective rate of the two treatment regimens was compared.Resu ts(1)Before treatment,there was no significant difference in each index between the two groups,which were comparable.(P>0.05).(2)After 28 days of treatment,ventilator monitoring data:VTi,VTe,and MV in the observation group and the control group were significantly improved compared with those before treatment(P<0.01);the indexes in the observation group were higher than those in the control group(P<0.01),and the difference was significant(P<0.05).Relevant parameters of blood gas analysis:PaCO2 and PaO2/FiO2 in the two groups were improved compared with before treatment(P<0.01),PaCO2 in the observation group decreased significantly compared with the control group(P<0.01),the difference was significant(P<0.01);PaO2/FiO2 in the observation group increased significantly compared with the control group(P<0.05),the difference was significant(P<0.05).Diaphragm mobility:bilateral diaphragm mobility in deep breathing was significantly increased in both groups(P<0.01);left diaphragm movement improvement in the observation group was significantly better than that in the control group(P<0.01),the difference was significant(P<0.01);right diaphragm movement improvement in the observation group was significantly better than that in the control group(P<0.05),the difference was significant(P<0.05).Respiratory symptoms score:respiratory symptoms score of the two groups were lower than before treatment(P<0.01);the improvement of the observation group was more obvious than that of the control group(P<0.05),and the difference was significant(P<0.01).Evaluation of curative effect:according to the evaluation of curative effect of respiratory symptoms,the overall curative effect of the observation group was better than that of the control group(P<0.01).The total effective rate of the observation group was 93.94%,and that of the control group was 75.76%.The effective rate of the observation group was higher than that of the control group(P<0.05).ConclusionAfter the "triple energizer acting on gas " acupuncture method combined with respiratory training,it can increase the tidal volume inhaled,exhaled and minute ventilation,reduce the partial pressure of carbon dioxide,improve the oxygenation index,increase the diaphragm mobility,and alleviate respiratory symptoms.This treatment has a good effect on the improvement of respiratory function in patients with tracheotomy after stroke.This treatment is safe,effective,economical and feasible.It has theoretical basis in both Chinese and Western medicine and is worthy of clinical promotion and application.
