ObjectiveTo analyze the effects and mechanism of combination of acupuncture with Chinese Medicine therapy (Blood-clearing and Anti-allergy Therapy of acupuncture and Chinese Medicine) on chronic urticaria, comparing with the western medication(loratadine) by small sample randomized controlled trial, in order to provide objective clinical evidences for its efficacy.MethodsChoose the inclusion of 60 patients with Chronic Urticaria, using completely randomized grouping method,60 cases were randomized to treatment group and control group with 30 cases in each. The treatment group was given acupuncture with Chinese Medicine therapy, medicines treatment for once a day and acupuncture treatment for five times a week. The control group was given loratadine once per day. The whole treatment process is in four weeks as a course. The outcomes were evaluated by Urticaria Activity Score (UAS), symptom rating scale of urticaria and Dermatology Life Quality Index(DLQI)at base line and after the whole treatment process. The clinical efficacy and correlation analysis is assessed by the SSRI of UAS. After the whole treatment, the relapse was observed in the follow-up visits in the 4th and the 8th week after the test. The data were processed with SPSS19.0 software.ResultsThe 60 patients that included study were randomly divided into treatment group and control group, with 30 cases in each.Among them, none was excluded and none was finally fall off, all the 60 patients completed the clinical observation. In comparison with the total curative effect in the treatment group, the total effective rate was 93.3%, control group total effective rate was 83.3%. The curative effect by rank test analysis, Z=2.84, p=0.005(P<0.05), with significant difference between two groups. After the treatment, UAS score of two groups after treatment were significantly decreased (P<0.01) and difference of UAS score of two groups compared with statistical significance (P<0.05). Among them, two groups of wheal score and pruritus scores were compared to the baseline decreased (P<0.01), the difference between the two groups of wheal score, pruritus score comparison showed that P values were less than 0.05, there was statistical significance. After the treatment, classification of urticaria symptoms scores of the two groups have improved significantly(P<0.01), urticaria symptoms score difference comparison showed that two groups of difference and the difference was statistically significant(P<0.05).After the treatment, the treatment group and the control group of patients with DLQI score decreased significantly than before(P<0.01).The difference of DLQI score compared, was statistically significant(P<0.05).The curative effect of two groups of different syndrome types of the ordinal variable group of four independent samples rank sum test, treatment group p=0.93(P>0.05), the clinical effect between no statistical significance; control group p=0.87(P>0.05). The clinical effect differences without statistical significance. After 4 weeks, treatment group 3 cases of recurrence, the control group of 5 cases of recurrence, by test p=0.45(P>0.05), the difference was not statistically significant. In 4 weeks after treatment, the recurrence rate of the two groups is almost the same. After 8 weeks, the treatment group,5 cases of recurrence, control group 12 cases of recurrence, P=0.045 (P<0.05), the difference is statistically significant. In 8 weeks, the recurrence rate in the treatment group than in the control group.ConclusionThe results of this study show, treatments of acupuncture with Chinese Medicine therapy and western medication can effectively improve the chronic urticaria symptoms and quality of life of the patients. And the treatment group in improving Urticaria Activity Score (UAS), symptom rating scale of urticaria and Dermatology Life Quality Index(DLQI), and treatment group can be effective in preventing the recurrence of chronic urticaria. This study confirmed that acupuncture combined Chinese Medicine therapy has a good clinical effect on the treatment of chronic urticaria. But this study in order to clear the Blood-clearing and Anti-allergy Therapy of acupuncture mainly, method combined with Chinese medicine dialectical theory of governance, evaluation and efficiency are better than western medicine group in the clinical efficacy. Clinical description method in the study of acupuncture combined Chinese Medicine therapy on chronic urticaria is significant, and no adverse reaction, provides a feasible method for the treatment of chronic urticaria, worthy of clinical popularization.
