Objective:This study mainly observed the expression level of hypoxia inducible factor-1 alpha (HIF-1α) and heat shock protein 70 (Hsp70), B cell lymphoma/leukemia -2 (Bcl-2) in the serum of patients with cerebral infarction at different points in time.We explored respectively the three factors that play the role in the course of ischemic stroke and the correlation between the three.Methods:Collected patients with acute cerebral infarction from neurology department of Sichuan province people’s hospital during February to November in 2013,and diagnosed by head CT or MRI examination of patients with cerebral infarction in middle cerebral artery blood supply zone for cerebral infarction group, a total of 32 cases, in the same period in Sichuan province people’s hospital of physical examination for control group, a total of 30.Two groups were elbow venous blood (about 2-3 ml), for the control group,we took blood once.For the cerebral infarction group, we took blood respectively at the onset of 12h,1d,3d,5d,7d,10d, all specimens after centrifugal take upper serum in the EP tube(eppendorf tube) after the tag in-80℃ refrigerator. By using sandwich enzyme-linked immunosorbent (ELISA) to detect the concentration of the serum HIF-1α, Hsp70 and Bcl-2.According to the 3 days after the onset of patients with head MRI results calculated the volume of cerebral infarction, divided into three groups:< 4.0 cm3 is small infarction group, a total of 7 cases;Between 4.0~10.0 cm3 volume of infarction group, a total of 14 cases;> 10.0 cm3 for large volume infarction group, a total of 11 cases.Application SPSS 19.0 statistical analysis software to analyze data, all measurement data are used to mean ± standard deviation (x±s), measurement data comparison between the two groups using independent sample t-test, count data comparison between the two groups using x2 test.The comparison of three groups mean using single factor analysis of variance, the mean of three group pairwise comparisons using LDS or SNK-q test.Correlation between two variables using bivariate correlation analysis, using the Pearson correlation analysis or Spearman rank correlation test.Results:1, the expression of HIF-1α(1) The serum HIF-1α in cerebral infarction group can be detected at the onset of 12h, and peaked in 1d.The expression of serum HIF-1α were higher significantly than the control group after the onset of 12h, Id,3d,5d,7d and 10d, the difference is statistically significant (p< 0.05).(2) Compared with expression of HIF-la in different volume of cerebral infarction:serum HIF-1α in large infarction group were significantly higher than middle infarction group and samall infarction group (p< 0.01).There was statistical difference in the middle infarction group and small infarction group after the onset of 12h, 1d,3d (p<0.05),middle infarction group showed no significant difference with small infarction group after the onset of 5d,7d and 10d (p> 0.05).2, the expression of Hsp70(1) The cerebral infarction group can be detected the expression of Hsp70 after the onset of 12 hours, and peaked in 1d, then gradually decline, the expression of Hsp70 has been close to the normal group level after the onset of 10d.The expression of serum Hsp70 were higher significantly than the control group after the onset of 12h, 1d,3d,5d,7d, the difference is statistically significant (p < 0.05).There has no statistical differences between the two group after the onset of 10d(p> 0.05).(2) Compared with serum HIF-1α in different volume of cerebral infarction:the expression of serum Hsp70 between large infarction group,middle infarction group and small infarction group had no statistical difference (p> 0.05).3, the expression of Bcl-2(1) The cerebral infarction group can be detected the expression of Bcl-2 after the onset of 12 hours, and peaked in Id, then gradually decline, the expression of Bcl-2 has been close to the normal group level after the onset of 10d.The expression of serum Bcl-2 were higher significantly than the control group after the onset of 12h, 1d,3d,5d,7d, the difference is statistically significant (p< 0.05),there has no statistical differences between the two group after the onset of 10d(p>0.05).(2) Compared with serum Bcl-2 in different volume of cerebral infarction:the expression of serum Bcl-2 in large infarction group were significantly higher than middle infarction group and small infarction group, there was statistical difference (p< 0.05). There was statistical difference in the middle infarction group and small infarction group after the onset of 12h, Id,3d,5d (p< 0.05),middle infarction group showed no significant difference with small infarction group after the onset of 7d,10d (p> 0.05).4, Application the double variable correlation analysis of the expression of serum HIF-la and Hsp70 in cerebral infarction group, the results show that r--0.184,p=0.314, suggesting they was no correlation between HIF-1α and Hsp70.Correlation analysis between serum HIF-1α and Bcl-2,the results show that r=0.726,p=0.000, prompt that they were positively correlated; Correlation analysis between serum Hsp70 and Bcl-2,the results show that r=-0.325, p= 0.07, prompt that they was no correlation between Hsp70 and Bcl-2.Conclusion:1, HIF-la, Hsp70 and Bcl-2 are involved in the pathophysiology process of acute cerebral infarction.2, There were positively correlated between HLF-1 a and Bcl-2, HIF-la involved that the reaction induced by hypoxia in acute stage of cerebral infarction, which induced transcriptional of anti apoptosis gene Bcl-2. Finally, there both play a neuroprotective effect.Hsp70 involved in effects of anti-apoptosis and anti-inflammatory, may also have a neuroprotective effect.3,The expression of serum HIF-1α and Bcl-2 may be have certain relations to cerebral infarction volume.It has important significance for evaluating the severity of the patients, may be one of the future biological indicators of judging prognosis.
