Research background: In recent years, with the rapid development of the national economy, traffic accidents and labor intensive industry are increasing. As a result, the cases of auto friction injury and machine twist rolling leading to limb skin avulsion are increasing. Its characteristic is large wound, heavy pollution, often associated with deep tissue, such as muscle, nerve, vessel and bone joint injury or combined with other organ injury. This kind of trauma is extremely difficult to treat. But along with microsurgical repair technology upgrading in our country, many methods and strategies in the treatment of this kind of trauma are proposed. The mainly treatment methods of skin avulsion injury are presented as follows: in-situ suture technique, strapping of operated chest wall, replantation of autologous skin, preserved subcutaneous vascular network skin graft, flap transfer and free flap transplantation, vacuum sealing drainge, replantation technique delayed with low temperature etc. At present, the above methods are still in the exploratory stage in clinical and experimental study, and a large number of clinical data is needed to support curative effect. In this experiment, microsurgical repair methods such as direct anastomosis, transpositional repair and cable grafting were used in the anastomosis of the vein and artery of avulsed skin from multiple-level, not only providing arterial blood supply but also ensuring venous patency. And this method restore the physiological anatomy characteristics of avulsed skin as far as possible, thus the survival rate increased greatly. Purpose: To investigate the effect of the next planting the skin under the microscope and the repair of rabbit model of anterior foot application on skin degloving injury. Methods: 48 New Zealand rabbits, by means of simulation tool cutting avulsion, crush fore foot scene, resulting in the big white rabbit forefoot degloving injury combined with tibia, metatarsal, finger(toe) bone fracture, the establishment of rabbit animal model of the foot skin degloved injury, 41 cases were successful, and the use of internal fixation of tibial fracture phalanx or repair of tendon rupture, under the microscope, the artery anastomosis front foot fracture and corresponding nerve, the dorsal skin and set off to the far side, through the direct anastomosis, anastomosed graft bridging method, such as by plane, arteriovenous anastomosis of dorsal, enable the skin to reset,a margin of time a margin of superficial vein anastomosis suture, skin edge, and placed the rubber drainage. Postoperative plaster fixation of rabbit model of the front foot, anti-inflammatory and anti-infection. Results: 41 cases of rabbit model of skin replantation survived, 3cases of wound edge skin a rabbit model of partial necrosis and healed by dressing. Postoperative X-ray review, phalanx-tibial fractures of good. The rabbit models were followed up for 3-9 months, average 5 months, fracture and skin wound healed well, the front foot can walk normally. Conclusion: In the microscope next planting the skin, the foot skin repair of degloving injury of the rabbit model, effectively restore the injured foot blood supply, accelerate the healing of fractures, walking function can effectively restore the injured foot in rabbit model.
