Objective1. To find information about the training status and needs of nurse attendants for the aged in Zheng Zhou;2.To investigate on the nurse attendants in elderly care felicities in Zheng Zhou, Henan Province, interview about their feelings about the training,and thus provide evidence for the construction of the training mode;3. To construct a standardized training mode for the nurse attendants for the aged of Zheng Zhou through Delphi method, based on the results of prevalence survey and qualitative research.MethodsThis research is divided into three parts.1.The investigation on the training status of the nurse attendants for the aged. From September 2014 to December, 76 qualified nurse attendants in the authorized elderly care facilities of Zheng Zhou were chosen as research subjects. After gaining support from corresponding elderly care facilities, investigators who had undergoneunified training completed the research on those 76 research subjects. Using general information questionnaire and questionnaire about the training status of nurse attendants for the aged as the research tool, relevant data are collected and analyzed using SPSS19.0 to get the means and standard deviations.2.Qualitative research on the cognitive level of nurse attendants for the aged. Using the research method of phenomenology, semi-structured interview with those nurse attendants was conducted. Using Colaizzi qualitative research method, those nurse attendants’ mastery of training knowledge was discussed and ways to improve their knowledge level through standardized training mode were put forward.3.29 experts who have been working in the nursing field for more than 10 years and know a lot about the training of nurse attendants were chosen, with their research areas being nursing education, nursing management and clinical nursing. Two rounds of consultation were conducted using the Delphi method.Results1. Investigation on the training status and needs of nurse attendants for the aged: 81 questionnaires were sent out and 76 questionnaires were available, with the effective rate being 93.83%. Within those 76 nurse attendants aged 24 ~ 63(35.50±9.14),women account for 85.53%(65/76),while men account for 14.47%(11/76). The nature of elderly care facilities: state-owned facilities account for 2.22%(1/45)while private-owned facilities account for 97.78%(44/45). 92.11% of those nurse attendants have gone though trainings held by their own organizaitons. 85.53%(65/76)own certificates for nurse attendants for the aged. 73.68%(56/76) have gone though trainings which last less than a month. 94.74%(72/76)regard the training for nurse attendants for the aged as a necessity. In the past trainings, daily care for the aged, the observation and nursing of common diseases for the aged, and laws and regulations concerning work ethic rank highest, with their rates being 92.11%(70/76)、90.79%(69/76)、86.84%(66/76)respectively. The training methods most expected by those nurse attendants are: study tours 96.05%(73/76)、lectures 90.79%(69/76)、experience exchanges 82.89%(63/76)、further education in other facilities 10.53%(8/76). 47.37%(36/76)believe the besttraining time should be around 1 week.2.The results of qualitative research:① the lack of standardized training mode;②inappropriate allocation of human resources;③ updating needs of the nursing concepts for the aged.3. The Delphi method:29 experts distilled 3 themes, with the first theme being indentifying the corresponding duties for nurse attendants at different levels in order to set up the level-to- level administration organization, the second theme being perfecting the continuous mechanism within the elderly care service and the key of the second theme being the continuity of information among elderly care facilities, and the third theme being providing enough resources(including personnel, facilities, land and so on) to guarantee the work of the elderly care facilities.4.The initial construction of nursing care training model:four level indicators: structures, training methods and training time, training contents and evaluation feedback. The composition of the structures include two secondary indicators:①staff responsibilities;②teaching environment and equipment. Training methods and training time include two secondary indicators:①classroom teaching, a total of 100 hours, including theoretical lectures, case studies and exchange of experience;②experimental teaching, a total of 40 hours, including the operating skills teaching, scenario simulation training and site visits. The training contents include six secondary indicators : ①safety knowledge; ②observation and evaluation of the functional status of clients; ③prevention and control of complications;④daily living abilities;⑤skills and methods of psychological care; ⑥health education. Evaluation feedback includes : ①evaluation feedbacks of clients;②self-evaluation.Conclusions1. Among nurse attendants for the aged, women are in the majority. Most of these women have gone though trainings held by their own organizations. However, the satisfaction rate needs improving.2. There is no current standardized training mode for nurse attendants for the aged. Allocation of human resources is inappropriate and the nursing concepts forthe aged need updating. The corresponding duties for nurse attendants at different levels should be identified in order to set up the level-to- level administration organization. The continuity of information among elderly care facilities should be maintained. Government should provide enough personnel, facilities, land and so on to guarantee the work of the elderly care facilities.3. The preliminary construction of the standardized training mode of nurse attendants for the aged includes 4 first class indexes and 10 second class indexes.
