| Objective: Observe the clinical effects of Shugantiaoxue Soup and Quyuzhixue Soup in treating endometrium irregular shedding of syndrome of qi stagnation and blood stasis, compare the level of the estradiol and progesterone in serum and the endometrial thickness pretherapy and post-treatment, and explore the possible mechanism involved.Methods: Eventually selected 60 cases of the endomentrium irregular shedding of syndrome of qi stagnation and blood stasis and divided into two groups randomly, in which the treatment group given Shugantiaoxue Soup and Quyuzhixue Soup, and the control group given Progesterone Capsules. Both should be under treatment for three menstrul cycles, and then collected fasting forearm venous blood and make gynecology ultrasonography in the morning before treatment and the third day of the third menstrul cycle under treatment respectively. Observe and record the clinical symptoms improved conditions and the level of the estradiol and progesterone in serum and the endometrial thickness pre-therapy and post-treatment.Results:1. The comparison of effects of the two groups: The total effective rate of the clinical comprehensive efficiency of treatment group is 83.33%, which of control group is 50.00%, and the curative rates of the two groups are 70.00% and 26.67% respectively. There is highly significant difference between them by the statistical analysis(P<0.01). The total effective rates of TCM syndromes clinical of the two groups are 93.33 % and 63.33%, and the curative effective rates of the two groups are 80.00% and 26.67%, and there are significant differences between them by the statistical analysis(P<0.05,P<0.01). The total effectiverates of cardinal symptom of the two groups are 93.33 % and 53.33%, and the curative rates are 66.67% and 30.00% respectively. There are highly significant differences between them by the statistical analysis(P<0.01).2. The comparison of TCM symptom average scores and the primary TCM symptom scores of the two groups: Compared with pre-therapy, there are significant differences in TCM symptom scores of the two groups after treatment and after drug withdrawal for three months. There are highly significant differences between the two groups(P<0.01). After drug withdrawal for three months, both of the two groups’ TCM symptom scores have risen again. For the treatment group, there is no significant difference between the scores of three-month-treatment and drug withdrawal for three months(P>0.05), however, for the control group, the differences are highly significant(P<0.01). For the two groups, there are highly significant differences in all of the primary TCM symptom scores after treatment compared with pre-therapy(P<0.01); there is no significant difference between the long-menstruation score of the two groups(P>0.05), and there are highly significant differences between another two primary scores of the two groups(P<0.01). For the treatment group, there are highly significant differences in all of the primary TCM symptom scores after drug withdrawal for three months compared with pre-therapy(P<0.01), and for the control group, the differences are also significant(P<0.01, P<0.05); there are highly significa-nt differences between the three primary scores of the two groups(P<0.01). For the treatment group, there is no significant difference in all of the primary TCM symptom scores after drug withdrawal for three months compared with post-treatment(P>0.05), and for the control group, the scores have risen again, and the differences are significant(P<0.01, P<0.05).3. The comparison of the menstrual days and the high temperature days of BBT before and after treatment: The menstrual days after three-month-treatment of the two groups are obviously shorter than pre-therapy(P<0.01); and there is no ignificant difference between the two groups(P>0.05).The menstrual days after drug withdrawal for three months of the two groups are also obviously shorter than pre-therapy(P<0.01),and there is highly significant difference between the two groups(P<0.01). For the treatment group, there is no significant difference between the days of three-month- treatment and drug withdrawal for three months(P>0.05); for the control group, the menstrual day has risen again, and there is highly significant difference(P<0.01). Both of the high temperature days of BBT of the two groups after three-month-treatment are shorter than pre-therapy(P<0.01); there is no significant difference between the two groups(P>0.05). Both of the high temperature days of BBT of the two groups after drug withdrawal for three months are also shorter than pre-therapy(P<0.01); but there is highly significant difference between the two groups(P<0.01). For the treatment group, there is no significant difference between the days of three-month-treatment and drug withdrawal for three months(P>0.05); However, for the control group, the high temperature day of BBT has risen, and there is highly significant difference(P<0.01). 4. The comparison of the level of the estradiol and progesterone in serum and the endometrial thickness of the third day of the menstrul cycle before and after three-month-treatment: The level of the estradiol and progesterone in serum of both groups are reduced obviously(P<0.01), and there is no significant difference between the two groups(P>0.05); both of the endometrial thicknesses of the two groups are obviously thinner than pre-therapy(P<0.01), and there is no significant difference between the two groups(P>0.05).Conclusion: Shugantiaoxue Soup and Quyuzhixue Soup have a good clinical effect on endometrium irregular shedding of syndrome of qi stagnation and blood stasis. They can improve the TCM symptoms obviously, and the clinical effect is stable after drug withdrawal. The mechanism is probably that they can regulate the function of the hypothalamus- pituitary- ovarian gonad axis’ and corpus luteum, and make the the corpus luteum atrophy on time in order that the endometrial fall off timely. By this way, the menses is shorter and we achieve the clinical purpose. |
