Objective1.To explore the clinical curative effect of Jianpichushifa on chronic urticaria; To compare the difference of clinical curative effect between Chushiweiling decoction and Desloratadine on chronic urticaria; and to develop the advantage of traditional chinese medicine on treating chronic urticaria.2. To study the serum levels of IL-17 in the patients with chronic urticaria.Methods60 outpatients, from the first hospital of traditional chinese medicine university Of Guang Zhou dermatology sexually transmitted diseases specialist out-patient, with chronic urticaria were randomly divided into two groups, each group had 30 cases. the control group took desloratadine 5 mg/d, and the observation group took desloratadine 5 mg/d combined with Chushiweiling decoction(1 dose/d).The two groups were treated continuously for four weeks together. Each patient had to return hospital for further consultation once a week, we recorded symptom score and adverse reactions. The clinical curative effects were appraised according to the points and the efficacy index before and after the treatment. two groups were determined the serum levels of IL-17 by ELISA before and after the treatment respectively. And the difference of the serum levels of IL-17 were compared between the two groups. In 4 weeks after treatment, patients were telephoned and inquired the presence of recurrence.Results1. In treatment group, the number of cure patients were 9, improved patients were 16 and relapse cases were 3. The curative ratio was 30.00%, effective rate was 83.33% and recurrence rate was 33.33%; In control group, the number of cure patients were 6, improved patients were 11 and relapse cases were 4. curative ratio was 20.00%, effective rate was 56.67% and recurrence rate was 66.67%.Comparisions of the curative ratio, effective rate and recurrence rate between two groups of patients after 4 weeks course were presented as follows, the curative ratio:x 2=0.80,P=0.37 (P>0.05), There was no difference between the two groups on curative ratio; the effective rate:x 2=5.08, P=0.02 (P<0.05), There was significant difference between the two groups; the recurrence rate: P=3.15(P>0.05), There was a significant difference between the two groups.2. Comparison of the symptom score before and after the treatment between two groups:there were such considerable differences on the wheal mount, itching, duration time and frequency(P<0.05), while there were no difference on the maximum diameter of wheal (P=0.74 P>0.05).3. Serum levels of IL-17 in the observation group were (22.4±4.90) pg/ml and (14.8±2.90) pg/ml before and after the treatment respectively, and the control group were (21.8±4.60) pg/ml and (19.13±3.71) pg/ml respectively. There was no difference between the two groups on the levels of IL-17 before the treatment, t=0.54P=0.59 (P>0.05). after the treatment, the levels of IL-17 in two groups declined,and the observation group declined faster compared with those in control group, there was significant difference in the level of IL-17 between two groups t=4.36 P<<0.01(P<0.05).4. There were no differences between the two groups on gender, age and course of disease.5. In treatment group,1 case patient in the process had the symptom of stomachache and 1 drowsiness and dizziness symptoms.2 cases patients in the process took to drowsiness and dizziness symptoms in control group.Conclusion1.The effect of Chushiweiling decoction combined with desloratadine in the treatment of chronic urticaria is better than desloratadine alone, and the clinical symptoms are improved effectively, with high security, and it s worth to be used popularly in clinic.2. Desloratadine is effective in the treatment of chronic urticaria, with less side effects.3. IL-17 may participate in the pathogenesis of chronic urticaria. The effects of Chushiweiling decoction in the treatment of chronic urticaria may contribute to IL-17.
