ObjectiveThe objectives of this study: to establish a novel and high efficient, according with the clinical reality, and patient-centered nursing intervention of coronary CTA; to explore the feasibility and effectiveness of Group visits Nursing Intervention on coronary CTA among Coronary artery heart disease Patients. Methods1. 120 patients in a third-level hospital were selected by using Random number table method in August 2014 to December 2014. All the patients were randomly assigned into intervention group and control group. The patients in the control group received conventional nursing, however, the patients in the intervention group received the health education of group visits in the conventional nursing foundation.2. In this pilot study, data were collected before the two groups intervention and after intervention, including blood pressure, heart rate, heart rate fluctuations, and breath-holding time. Image quality and the success ratio were also recorded. We designed the record charts of observation target by ourselves. Before intervention, the patients in the present study should write Coronary heart disease(CHD) health knowledge questionnaire and self-rating anxiety scale. All the CTA scan was operated by the same mechanic make operator.3.In the certain designated places, researchers will explain the purposes and meaning of the survey. The record charts of observation target, CHD health knowledge questionnaire and self-rating anxiety scale were distributed by researchers before and after intervention. The patients’ blood pressure, heart rate, heart rate fluctuations, and breath-holding time were recorded. The difference of the scanning time, image quality and the success ratio were compared among groups.4.All images data were analyzed by using a software package(SAS, version 17.0). A value of P<0.05 was considered statistically significant difference. Descriptions were presented as means and standard errors of the mean, and categorical variables were presented as frequencies or percentages. T-test or chi-square test was performed to compare the differences of various factors before and after intervention. Results1. Before intervention, two groups have no difference in statistical significance(P>0.05).2. Before the group visits nursing intervention, the average blood pressure was(140.70±11.38) mm Hg, the average heart rate was(63.56±3.67) time/min, the average breath-holding time was(30.97±11.83) s, the average heart rate fluctuations was(4.22±0.41) time/min. There was a significant difference between the two groups, except for heart rate.3.Before the group visits nursing intervention, the physiological and psychological reaction of the patients in the intervention group response was more dramatic, in which increased blood pressure, heart rate increase fast and anxiety was dominant compared with other physiological and psychological reaction. There was a significant difference between the two groups after the group visits nursing intervention, except for the number of increased blood pressure patients.4.Most CHD patients failed to reach the diagnosis of anxiety. However, the SAS standard score before the group visits nursing intervention is obviously lower than that after the group visits nursing intervention. It may suggest that the group visits nursing intervention can remit strain of patients affected by coronary CTA.5.There was a significant difference between the two groups in learning CHD health knowledge questionnaire.6.Compared with the scanning time and the success ratio in the control group, the patients in the intervention group has a apparently shorter scanning time and a higher success ratio.7.After the group visits nursing intervention, the image quality of 56 cases(93.33%) in the intervention group by primary coronary CTA has reached 3 pointer, while the number patients in the control group was only 48(80%). There was no significant difference between the two groups. ConclusionThe group visits nursing intervention is a novel and high efficient, according with the clinical reality, and patient-centered nursing intervention of coronary CTA. It can improve the quality of nursing and use the health resources fully, improve the quality of the image, the success rate and the patients’ degree of satisfaction.
