Organ transplantation is a landmark biomedicine technology in20thcentury, it has brought about revolutionary changes in the field of medicine.In recent years, with the development of medical surgical techniques, theimprovement of organ preservation technology and the advent of newimmunizations, organ transplant surgery is increasingly becoming thestandard of surgical therapy, this technology not only brings hope to manypatients with end-stage, but also reflects the relevant level of hospital andmedical complex of the country. Many countries face the serious shortageof donor organs, so how to distribute the limited number of donor organs,and how to ensure a fair and efficient distribution of organs, is a veryimportant event in human organ transplant system.The existing domestic organ research is just about the discussion offairness and effectiveness, how to increase organ donation sources andhow to pass legislation to regulate organ transplantation, but there is nospecific discussion on organ transplant distribution model. The existingorgan allocation models abroad are only based on static patients’state. Thisarticle takes into account of the patient’s current health state as well as theimpact of future changes, we first set up a liver allocation model based onutility, and use the duality equations to get the solution. Then we point outthat fairness is peoples’ psychological experience, and use compensationutility to express the unfairness, trying to get a balance between fairnessand utility. Finally we get a rank allocation result and use the ExtendSimsimulation model to compare the different results between our allocationpolicy and the current liver allocation policy.
