BackgroundCancer pain is one of the most common symptoms in cancer patients, severe pain when seriously affect the treatment and quality of life of patients, drug treatment with opioid represented often appear some side effects and addiction, and prone to tolerance. Acupuncture in the treatment of cancer pain has been widely used in clinical, good curative effect and little adverse reaction, and even can be long-term use of adverse reaction caused by pain medication prevention and treatment for patients with cancer pain, also has a role in regulation of depression in cancer patients. Therefore, in the treatment of cancer pain has played an increasingly important role.ObjectivesThis topic through the acupuncture combined with level three ladder analgesic method in the treatment of severe cancer pain and simply use the three ladder analgesic therapy were compared, to observe its efficacy and safety evaluation of the pain relief, and the effect on depression, quality of life.MethodsSelect60patients met the inclusion criteria for patients with moderate to severe pain from oncology, Department of traditional Chinese medicine, Nanjing Gulou Hospital, were randomly divided into treatment group and control group, the treatment group of30cases,30cases in the control group. The control group using three ladder analgesic therapy, mild pain, NRS score of4-6points, the use of tramadol sustained-release tablets (100mg/tablet) treatment, one at a time, Q12H; severe pain, NRS score of7-10points, the use of oxycodone hydrochloride controlled-release tablets (10mg/tablet) treatment, one at a time, Q12H,7days for a course of treatment. Based on the above treatment group treated with acupuncture, the main acupoints are Hegu and Neiguan, and according to the primary disease and pain with the corresponding acupoints and back Shu Points, once a day,30min each time,7days for a course of treatment. The average NRS score to record every day, through the contrast before and after treatment in patients with pain, pain onset time, duration of pain relief, the adverse reaction during the treatment (nausea, vomiting, loss of appetite and so on), depression and the changes of tumor patient quality of life score. ResultAfter treatment, the two groups were compared, there were no significant difference (P>0.05), significant efficiency had significant difference (P0.05), the two groups are all can relieve the pain, but the treatment group is better than control group.2. After treatment, the two groups were compared, the effective pain relief time, pain relief continued significant difference (P<0.05), that has a faster onset of pain relieving treatment group, longer duration.3. After treatment, were compared between the two groups, the incidence of adverse reactions were significantly different (P<0.05), to a certain extent that acupuncture can relieve the pain of patients with gastrointestinal adverse reactions caused by the use of analgesic drugs.4. After treatment, the two groups were compared, there is significant difference in the score of life quality of patients with cancer pain(P<0.05), description of analgesic drugs can improve the life quality of patients with cancer pain with acupuncture.5. Before and after the treatment, the SDS self rating scale was used for assessment, treatment two group can improve the depressive state of patients, the difference was statistically significant(P<0.05); the treatment group compared with the control group, no significant difference (P>0.05).ConclusionEffect of acupuncture combined with western medicine in the treatment of severe cancer pain clinical analgesic effect, exact, quick effect, and long duration, less gastrointestinal adverse reactions, improve the quality of life of patients with cancer pain significantly, and can improve the depressive state of patients in a certain extent, is the most ideal choice of patients with cancer pain.
