Background and purposeEpithelial ovarian carcinoma is one of common malignant tumor in department of gynaecology and the mortality rate among the tumors of the gynaecology is the top one. In the early time, there is no obvious clinic symptom and the way to detect the tumor marker is so limited that when we detect it, it is almost advantaged stage. If we can detect the disease in the stage I,90percent ovarian cancer will be cure. In fact, only less than25percent ovarian cancer has been found in stage I, so it has a huge value to find to a way which is easy、accurate and safe,to detect the ovary cancer in early stage. There are some researches about the HE4、OPN、C-OPN and found they are all relate to the metastasis and outcome of the ovarian cancer. In the text, we explore the expression the levers of HE4、OPN、C-OPN in the ovarian cancer tissue、ascites、serum and urine (24hours before and one week after the operation) and to discuss the diagnostic value to the epithelial ovarian carcinoma.Material and methods1object and study:From September2010to March2012,98Patients with ovarian cancer,32patients with benign ovarian tumors,25Patients with normal ovary, all patients were hospitalized in the First Affiliated Hospital of Zhengzhou University. The serum and urine of the control group are from the women who do physical examination in the same period,and tissue which is confirmed by pathology in mormal tissues is form the patients who undergo the ovariectomy because of the multiple myomata of uterus or cervical cancer.All the patients never have any other tumor history、radiotherapy and chemotherapy treatment before and the disease must be diagnosed according to the pathology. The meso-position age of the epithelial ovarian cancer patients is46,among the cancer patients there are49cases serous,22cases mucous, and27cases other kinds of epithelial ovarian cancer. There are23cases in stage Ⅰ/Ⅱ,75cases in stage Ⅲ/Ⅳ,31cases in not postmenopausal stage,67cases in postmenopausal,27cases combined with ascites,18cases serous ovarian cancer,8cases mucous ovarian cancer, endometrioid carcinoma1case; The meso-position age of the benign ovarian tumor patients is43,among the patients, there are16cases serous,16cases mucous; the serum and urine of the control group are from the women who do physical examination and the meso-position age is42, the,and tissue is form the patients who undergo the ovariectomy because of the multiple myomata of uterus. Tissue is form the patients who undergo the ovariectomy because of the multiple myomata of uterus and the meso-position age is50.All the patients have signed contract and is according to the required of the medical ethics.2methods:ELISA method was used to investigate the expression levels of HE4and OPN in serum, urine and ascites and to investigate the expression levels of C-OPN in urine and ascites. RT-PCR and Western were respectively used to investigate the mRNA and protein of the HE4, OPN and C-OPN.3Statistical analysis:The SPSS statistical package program17.0was used. Production receiver operating characteristic (ROC) curve.Measurement data were expressed as mean±standard deviation.Comparison between the two groups with t-test.The group of before and after surgery using paired t-test. Between groups were compared using analysis of variance, variance missing time by rank sum test, with a =0.05level as a statistical test, P<0.05was considered statistically significant.Result1The expression levels of mRNA and protein of HE4、 OPN and C-OPN in the ovarian cancer、benign ovarian tumor and normal ovarian tissue.The expression levels of mRNA and protein of HE4and OPN in the cancer group are higher than the benign and normal groups. stage Ⅲ/Ⅳ are higher than stage Ⅰ/Ⅱ. Postmenopause group is higher than the non postmenopause group. The difference all have statistic significance(P value<0.05); there is no expression of C-OPN in the three group. The expression of HE4mRNA and protein in serous ovarian cancer group is higher than the mucous group and the difference between the two groups has statistic significance(vPalue<0.05).There is no difference of OPN mRNA and protein in two groups.2The expression levels of HE4、OPN and C-OPN in the serum and urine in the different groups2.1The expression levels of HE4in the cancer group are higher than the benign and normal groups. The difference between the two groups have no statistic significance(P value>0.05). The expression levels of OPN in the cancer group are higher than the benign and normal groups. The difference between any two groups have statistic significance(P value<0.05). The expression levels of C-OPN in the cancer group (25.24±7.04) ug/1in urine are higher than the benign group (10.31±2.81) ug/1and normal group (11.46±4.02) ug/1and the difference between the groups have statistic significance but the difference between the benign and normal groups have no statistic significance(Pvalue>0.05).2.2The expression of HE4in ascites、serum、urine in serous ovarian cancer groups are higher than the mucous groups and the difference between the two groups has statistic significance(Pvalue<0.05). In ascites、serum、urine, there is no difference of OPN in two groups. C-OPN expression level in the urine of serous ovarian cancer patients are higher than in the mucinous ovarian cancer patients, the difference has statistic significance and there is no obvious difference in ascites in the two groups.2.3The expression levels of each factor in serum and urine of24hours Preoperative are higher than a week after surgery and the serum ovarian cancer group decline faster. stage Ⅲ/Ⅳ are higher than stage Ⅰ/Ⅱ. Postmenopause group is higher than the non postmenopause group. The difference all have statistic significance.2.4HE4and OPN in Serum and urine has a Moderate consistency with the Pathologic findings if the respective critical value as the diagnostic criteria. C-OPN in urine has a High degree of consistency with the Pathologic findings if the critical value as the diagnostic criteria.Conclusion HE4and OPN were highly expressed in ovarian cancer tissues and stably secreted into serum,ascites and urine. C-OPN was fragments of OPN. The detection of tumor markers in urine may be better than serum.
