Objective:Ovarian endometriosis cysts is a common type of endometriosis,that mostly occurs in women of reproductive age,can lead to infertility,and even cause chronic pelvic pain,seriously affecting patients Of life quality.Laparoscopic cystectomy of ovarian endometriosis is the main treatment,at the same time,the clinical use of GnRH-a(gonadotropin-releasing hormone agonist)drug adjuvant treatment of the disease.Ovarian endometriosis cysts is a surgical treatment,or conservative treatment of drugs,preoperative diagnosis and differential diagnosis is very important,especially for the recent fertility requirements of women.More and more domestic and foreign experts have done a lot of related research on the Ovarian endometriosis cysts diagnosis and differential diagnosis.The aim of this study was to evaluate the value of serum CA125,CA199,HE4 in the preoperative diagnosis and differential diagnosis of the disease.In order to understand the degree of difficulty of operation,it is necessary to determine whether preoperative and postoperative GnRH-a drug therapy.Methods:Retrospective analysis and comparison with surgically confirmed ovarian endometriotic cysts disease397 cases(patients group),72 malignant ovarian tumor patients(case-control group A),87 benign ovarian tumor patients(Only contains ovarian serous and mucinous tumors,case-control group B)in the blood of three tumor market test results.And to analyze the sensitivity and specificity of three kinds of tumor markers in the diagnosis of ovarian endometriotic cyst,and the significance of differential diagnosis in endometriotic cyst.Rusults:The median serum levels and positive rate of CA125,CA199 and HE4 for ovarian endometriosis cysts were :38.80U/ml ? 62.72%,32.39U/ml ? 45.34%,37.20pmol/L?0.50%.The median serum levels of CA125,CA199 and HE4 for malignantovarian tumor were:136.40U/ml,15.58U/ml,89.20pmol/L.The median serum levels of CA125,CA199 and HE4 for benign ovarian tumor were:12.20U/ml,13.06U/ml,35.40pmol/L.The sensitivity and specificity of serum CA125,CA199 and HE4 for ovarian endometriosis cysts were 62.70% ? 61.00%,45.30% ?79.20%,0.50% ? 74.80%.The median serum levels of CA199 and HE4 were significantly lower in patients group than those in case group A(P <0.001),the difference was statistically significant.The median serum levels of CA125 and CA199 in patients group were significantly higher than that of case control group B(P <0.001),the difference was statistically significant.The median serum HE4 level in the control group A was statistically significant compared with the diseased group(P <0.001).The median CA125,CA199 and HE4 levels in the control group A were significantly higher than those in the control group B(P <0.001).Conclusion:Serum CA125,CA199 levels can assist in the diagnosis of ovarian endometriotic cysts,Three serum tumor markers combined diagnosis can improve the diagnostic sensitivity.The use of serum CA125 and CA199 differential diagnosis of ovarian endometriotic cysts and benign ovarian cysts has significant significance.Although HE4 for the diagnosis of endometriosis cysts had no meaningless,but for the differential diagnosis of malignant ovarian cysts have obviours meaningful.
