ObjectivesTo establish the preliminary formation of nurse stratified, access conditions, jobresponsibilities in GradeⅡand III hospitals of Henan province, to provide referencefor establishing a available and usable system of nurse stratified and promoting thedevelopment of the nurse post management.MethodsQualitative research and quantitative research were Combined.The first step,Findand read the literatures in Chinese and other languages, according to the clinical nursefive ladder mode and the corresponding theoretical level, the construction of apreliminary draft management index system on the layer classification of the nursewas made. The second step, through the phenomenological method for extraction ofsecondary and tertiary care hospital in Henan province personnel (hospital director,the director of nursing department, the head nurse, nurse) individual in-depthinterviews, on the basis modify the draft. The third step, using the nursing collegegraduate dissertation proposal of experts to discuss machine, further modify the indexsystem. The fourth step, adopting Delphi consulting method, select20nursing expertsof first-tier cities, second-tier cities in Henan province to evaluate the initial formation of the index entries. Using SPSS17.0software for statistical analysis.ResultsQualitative research part was extracted three related topics, respectively:①Theadvantages and disadvantages of hierarchical management to nurses. Most nursesthink that the biggest beneficiaries are patient, nurses classified is conducive to theoverall nursing for patients; All the interview objects consider that nurses layerclassification management is beneficial to the nurse’s career development and thestability of nursing team.②Knowledge and opinions on the layer and classificationmanagement of nurses. The most importment thing is setting nurses hierarchical levelstandards.③Obstacles on the layer classification o f the implementation of nursingstaff in Henan province.Human factor is that the hospital scale expansion is fast, theprofessional title and age structure of nurses is not reasonable. These studies based onthe layer classification of the localization of the nurse in Henan province providespractice basis to building the management index system of nurses locally.Expert consulting results. The positive factor of experts consultation In the firstround is: recovery rate is90.9%, recommended rate is90%, authority coefficient is0.885, Item importance mean score ranges1.65~5.00, the variation coefficient is0.00~0.69,the average coordination coefficient is about0.193;The statistical result of the second time expert consultation is: the positivecoefficient and recovery rate100%, recommended rate35%, authority coefficient is0.892, Item importance mean score ranges4.30~4.95, the variation coefficient0.045~0.212. Average coordination coefficient is0.431.Two rounds of expert opinioncoordination degree through chi-square test, p value is less than0.05, expert opinioncoordination degree is high.Initially index framework of nurse stratified in GradeⅡand III hospitals ofHenan province was formed after two rounds of expert consultation. Clinical nurseswere divided into the base layer, growth layer, backbone layer, professional layer, andthe expert layer, to correspond to the five classes: N0assistant nurses, N1registerednurse, N2primary nurse, N3specialized nurses/Nursing managers, N4clinical nursing specialist/nursing researchers/Nursing management experts. Various levels ofnurses have corresponding requirements and responsibilities framework.ConclusionThe representation and credibility of the object was to interview and consultingexperts of this study is high, nurse layer classification standards and the requirementsand responsibilities index system built in the secondary and tertiary hospitals inHenan province has good concentration,it is expected to provide decision-makingreference for administrative department responsible for health and hospitals innursing human resource management.
