The Study On Clinical Nurse Stratification System Of Grade Three General Hospital In Henan Province

Background Since the1990s, the reasonable allocation of nursing staff influence on the quality of care is more and more obvious, following the rapid development of Chinese nursing career and foreign advanced nursing theory and method continuously introducing, In addition, with the nurses management regulations of the People’s Republic of China is issued and high quality nursing service engineering in an all-round way, people put forward a higher demand in the nursing work, China mainland have not form a clear unified nursing staff stratification system, does not have a set of forming nursing staff access rules.Objective This study intends to set up a scientific, objective, comprehensive of the clinical nurse stratification system for grade three general hospital in Henan province, to change the present structure of nursing human resources, and to establish a clear development goals for clinical nurse career development at all levels, provide the patients with a high level service.Methods Two rounds of questionnaires consult for31nursing department directors from different grade three general hospital in Henan by the Delphi method process. Questionnaire mainly includes three parts:tribute and guide language; Experts basic information questionnaires and experts authority degree questionnaire; the nurse stratified evaluation index. In the first round of questionnaire survey, the experts must fill out personal information and authoritative information degree, and the system of the important degree at various levels and the index weight give advice, if there are different views, can increase, delete or modify item in blank space. Inductive, sorting, analysis of the data after the first round of questionnaire back, according to the evaluation index important degree and the modification of the sorting weight. In the second round of expert advice, the experts expression or modify the result by secret ballot, the experts confirmed that layered index important degree, all levels weight,work content, and proposed own opinion. Then according to expert’s advices and to determine the indicators of the clinical nurse stratified using index system. Determined the weight and all levels of nurse working content of the clinical nurse stratified using system in grade three general hospital in Henan province, and test its logical consistency.Results The effective recovery rate of two rounds of questionnaires had reached more than90%, the coefficient of authority of experts was above0.70±0.09, between [0.55,0.90] interval, within the scope of this subject of expert consultation system established authority effective; While the consistency test of index system meet the requirements of the logical consistency, which indicated the weight of all the index were scientific and objective. According to the consultation, operation in three steps with elements decomposition level quantitative method:Firstly, decomposition index evaluation factors at all levels; Step two, determine the score a value; Step three, index grade standards. Establish the nurse stratified use system:first, named all levels nurse, then formulate the formula; Final, determine score of all levels nurse. Finally established5first-class indexes,7second-class indexes and all levels of nurse working content, which consist the index system of clinical nurse stratified using in grade three general hospital in Henan province.Conclusion The clinical nurses stratification system is scientific, complete, feasible and operate easy, which will promote clinical nurses stratified use and high quality nursing service carrying out.
