| ObjectiveAccording to the study which has been completed that abdominal acupuncture treat the pelvic pain caused by endometriosis, we have concluded:site selection of abdominal acupuncture using Yin Qi Gui Yuan and Zhongji as the basic prescription, and choosing points according to the site of pain can effectively alleviate the pelvic pain caused by endometriosis. On the base of that study, this study explore further the clinical efficacy of dialectic of abdominal acupuncture point selection in the treatment of dysmenorrhea caused by endometriosis, through the comparison between groups, analyze the difference of the clinical efficacy between the treatment group and the control group, accumulate data for the next randomized controlled trials in the treatment of dysmenorrheal due to endometriosis by dialectical selection of abdominal acupuncture.In addition, this study also observed the impaction of the two kinds of abdominal acupuncture point selection method for serum CA125level, EmAb, volume of ovarian chocolate cysts and the sum of the three diameters of the uterus; observed the clinical efficacy of the two kinds of abdominal acupuncture for different syndromes and different levels of dysmenorrhea.MethodsThis study used non-randomized studies, the100cases of endometriosis who with the major purpose of dysmenorrheal were collected from October2011to January2014at the department of gynecology in Guangdong Provincial Hospital of TCM, they were divided sequentially into the treated group using the dialectical selection of abdominal acupuncture with50cases and the control group using the location selection abdominal acupuncture with50cases. Both groups were treated three menstrual cycles. Respectively, before the treatment, after completion of each cycle of treatment and the next menstrual clean after all of the treatment, patients wrote the visual analog pain score and tables of integrals with dysmenorrheal. Before and after treatment, they were checked serum levels of CA125, EmAb, take gynecologic examination and ultrasound. Through the comparison of the scores of visual analog pain score and tables of integrals with dysmenorrheal symptoms, volume of ovarian chocolate cysts, the sum of the three diameters of the uterus and serum CA125level and so on before and after the treatment, analyse clinical efficacy of the dialectical selection of abdominal acupuncture.Results1. After the treatment, the VAS scores and the points of the tables of integrals with dysmenorrheal of the two groups were decreased, and the differences were statistically significant (P<0.05). Between the two groups differences were also statistically significant (P<0.05)。In the overall efficacy, the study group has better clinical efficacy than the control group, showed that the analgesic effect of the dialectical selection of abdominal acupuncture is better than the location selection abdominal acupuncture.2. For patients with ovarian chocolate cysts, after the treatment, their volumes of ovarian chocolate cysts were reduced, and the differences were statistically significant (P<0.05) in the two groups, while the difference between the two groups was not statistically significant (P>0.05), showed that abdominal acupuncture can reduce the volumes of ovarian chocolate cysts, while the effect is the same of the dialectical selection of abdominal acupuncture and the location selection abdominal acupuncture. For patients with adenomyosis of uterus, after the treatment, the differences of the total of the three diameter line of the uterus were statistically significant (P<0.05), while the difference between the two groups was not statistically significant (P>0.05), showed that abdominal acupuncture can reduce the total of the three diameter line of the uterus, while the effect is the same of the dialectical selection of abdominal acupuncture and the location selection abdominal acupuncture. 3. After the treatment, the serum levels of CA125of the two groups were decreased, and the difference was statistically significant (P<0.05), while between the two groups difference was not statistically significant (P>0.05), showed that both groups can decrease the level of CA125, while the effect is the same.4. The points of the tables of integrals with dysmenorrheal reduced with different syndromes of TCM in both groups, compared before and after treatment, the differences were statistically significant (P<0.05), and each syndrome of TCM between the two groups, difference of the type of stasis.by cold was statistically significant (P<0.05), differences of the other three were all P>0.05, showed that the dialectical selection of abdominal acupuncture and the location selection abdominal acupuncture have analgesic effect, the analgesic effect of the study group is better than the control group for the type of stasis by cold. After the treatment, the points of the tables of integrals with dysmenorrheal of different extent of dysmenorrhea in both groups were decreased, and the differences were statistically significant (P <0.05), and the same level of dysmenorrheal between the two groups, the difference was also statistically significant (P<0.05), showed that the analgesic effect of the dialectical selection of abdominal acupuncture is better than the location selection abdominal acupuncture for moderate and mild dysmenorrhea.ConelusionThe dialectical selection of abdominal acupuncture has better clinical efficacy than the location selection abdominal acupuncture. The analgesic effect of the dialectical selection of abdominal acupuncture is better than the location selection abdominal acupuncture. Both can improve endometriosis, decrease serum CA125, and the effect is the same. Both have analgesic effect, the analgesic effect of the dialectical selection of abdominal acupuncture is better than the location selection abdominal acupuncture for the type of stasis by cold. The analgesic effect of the dialectical selection of abdominal acupuncture is better than the location selection abdominal acupuncture for moderate and mild dysmenorrhea. The dialectical selection of abdominal acupuncture is more suitable than the location selection abdominal acupuncture for patients with endometriosis dysmenorrhea, can relieve pain to a greater degree, while improving endometriosis, decreasing serum CA125levels. |
