Osteosarcoma is an idiopathic, highly malignant tumor, it derived frommesenchymal tissue, good hair at20years old of the following young men andchildren, mostly on the long bone metaphysis, early metastases, high mortalityclinically, seriously endangering the health of teenagers. Before the70’s of thetwentieth Century, or even amputation radiotherapy is the major treatment ofosteosarcoma, although temporarily can develop in the local control ofosteosarcoma, but according to statistics, up to80%after treatment appearedto shift away from the site of the patients, overall survival rate is only about20%; by the age of80, neoadjuvant chemotherapy (neoadjuvantchemotherpy)was established to implement concept, advocated the biopsy diagnosis ofosteosarcoma after chemotherapy immediately, at the same time, according tothe scheme of tumor necrosis rate of postoperative chemotherapy; in the past30years, with the continuous improvement of neoadjuvant chemotherapy forosteosarcoma and wide application, the5year survival rate reached about60%. The main concept of treatment is now at home and abroad:comprehensive treatment of osteosarcoma, surgical operation is still the maintreatment method, according to the different condition of feasible limb salvageoperation, at the same time, the effective adjuvant chemotherapy regimens, thecombination of the two clinical application can significantly improve theprognosis of patients with osteosarcoma.Although the past two years operation resection and adjuvantchemotherapy technology unceasing development, effectively reducing themortality rate of osteosarcoma, but there is still30%patients even aftertreatment may appear in the process of metastasis or recurrence, once thediagnosis of distant metastasis, the patient’s5year survival rate is only20-30%; tumor cell resistance to chemotherapeutic drugs; used duringchemotherapy drugs such as cisplatin, carboplatin, methotrexate, cyclophosphamide, glucocorticoid, paclitaxel and vincristine can causedifferent degrees of abnormal glucose tolerance, even induce diabetes, so thatchemotherapy can not proceed, impact on patient outcomes and quality of life.These factors lead to failure in treatment of a variety of means can be used inclinic. People on the treatment of osteosarcoma has been transferred to thefield of gene, has to explore the application of antisense gene, tumorsuppressor genes, immune gene, target gene, Dutch act gene therapy, genetherapy for this tumor is a promising, become the hot clinical cancer researchat present.CD147is the extracellular matrix metalloproteinase inducer (EMMPRIN),can stimulate tumor surrounding fibroblast cells to produce MMPs, createfavorable conditions for the invasion and metastasis of tumor, it is a kind ofcell surface adhesion molecules, present studies have found CD147andangiogenesis of tumor have a close relationship, think CD147can promotetumor angiogenesis, and create conditions for the proliferation and metastasisof tumor cells. Research has found that the expression of CD147and MMP-2in various malignant tissues increased, play a very important role in theinvasion and metastasis and angiogenesis of tumor, and the pathological typesand prognosis is correlated with tumor.Objective:Through the determination of CD147and MMP-2expression inosteosarcoma tissues, to explore the development and function of the twoinvasion and metastasis in hydatidiform mole, interrelated and among the two,to determine the combined detection of CD147and MMP-2will become anew index to judge the prognosis of osteosarcoma.Methods:Specimen collection:Collect the months of2003-2013year in May, the Affiliated Hospital ofChengde Medical College, Baoding second hospital, Hebei MedicalUniversity third hospital operation resection specimens from37cases ofosteosarcoma,10cases of osteochondroma specimens, were tested by two clinical pathologist verification. The tissues were collected with4%neutralformaldehyde soaking, rinsing,100%,90%,75%of the gradient alcoholdewatering, xylene, embedded in paraffin. Continuous paraffin section, thethickness of0.5um.Grouping:The organization is divided into osteosarcoma group (37cases), bone andcartilage tumor group (10cases); osteosarcoma group according to the genderis male (20), female (17) of two groups, according to the histological type isdivided into osteoblastic type (17), fibroblast cell type (7), chondroblastoma(9), the other (4); according to Enneking staging system divided into phase I(4), stage a (7), ⅡB (14), III (12), with or without pulmonary metastases weredivided into two groups: a group (23), lung metastasis no pulmonarymetastasis group (14).Test method:The37cases of osteosarcoma,10cases of osteochondroma tissueswere immune markers were detected by immunohistochemical S-Pmethod,statistical, analysis was performed on the expression of CD147andMMP-2in two groups.Detection method:Each slice of randomly selected5horizons, the immunohistochemicalresults according to the staining degree and positive cell percentage score, toproduct staining degree score and positive cell percentage in the total scorecalculation.Statistical analysis:Application of SPSS17.0statistical software for statistical processing,compared with the sample rate of the chi square test, P <0.05illustrate thedifference had statistical significance. The correlation between MMP-2,CD147by Spearman test, P <0.05illustrate the difference had statisticalsignificance.Results:1Immunohistochemistry results showed that: the cell membrane and cytosolic CD147, MMP-2positive products were located in osteosarcomacells, brown granule like material, interstitial and normal tissue without orwith little coloring. The positive expression of CD147in osteosarcoma was ashigh as89.2%, while the positive rate of the control group of osteochondromais0, the difference was significant (P <0.01). The positive expression ofMMP-2in osteosarcoma was up to78.4%, while the positive rate of thecontrol group of osteochondroma is only20%, with significant difference (P <0.05)2The positive expression of CD147,MMP-2in osteosarcoma cells withage, gender and Dahlin tissue typing independent (P>0.05). Two the positiveexpression in osteosarcoma and the clinical staging of Enneking and nopulmonary metastasis (P <0.01): with the increase of osteosarcoma clinicalstage, the positive expression of CD147and MMP-2rate also increasedgradually; the positive expression of lung metastasis group CD147andMMP-2were significantly higher than those without metastasis group.3In osteosarcoma, there is a certain correlation between the expressionof CD147and MMP-2, were positively correlated (r=0.451, P <0.01).Conclusion:1Osteochondroma dose not express CD147, the positive expression ofCD147and MMP-2in osteosarcoma group was significantly higher than thatof bone and cartilage tumor group, and with the increase of clinical staging oftumor Enneking, CD147and MMP-2expression levels were also elevated;positive expression and pulmonary metastasis in osteosarcoma patients inCD147group, MMP-2was significantly no pulmonary metastasis group washigher than that of the two, the transfer and expression of osteosarcoma andmalignant degree.2Positive expression of CD147and MMP-2in osteosarcoma, suggestingthat the two in a synergistic effect in the occurrence and development of tumor.So, CD147and MMP-2positive expression is an important symbol of avariety of malignant tumor with poor prognosis, the combined detection ofCD147and MMP-2is expected to become a new index for judging the prognosis of osteosarcoma.
