[Objective] By analyzing the case history, mental health and relevant factors of132inpatients suffering from Type2Diabetes in third elderly internal medicine department of Second Affiliated Hospital of Kunming Medical University, evaluate the mentality and social life level of Type2Diabetes patients with insulin resistance, and then discuss the relation between insulin resistance patients and the depression incidence and the relevant risk factors, so as to provide efficient basis for disease control, mental intervention and complication relieving of diabetes.[Methods] Collect132cases of Type2Diabetes patients in third department in our hospital from Aug.2012to Oct.2013, among which, there are87cases of patients with insulin resistance and45cases of patients without insulin resistance. There are71female patients,61male patients. Use Hamilton’s Depression Scale (HAMD) questionnaire to grade the depression level and group them. Evaluate the life quality satisfactory degree of diabetes patients by quality of life instrument (DQOL). The questionnaire will also collect patients’age, gender, education degree, salary, related biochemical supersession indexes (uric acid, cholesterol, triglyceride. fasting blood-glucose, glycosylated hemoglobin, and fasting insulin) and some general material about diabetes. Calculate the occurrence rate of depression, and compare the occurrence rate of patients with insulin resistance and that of patients without insulin resistance, then select the risk factors of depression in the way of logistic regression.[Result]1. The depression rate of patients with type2diabetes insulin resistance has reached41.5%in this study while that of patients without insulin resistance is22.2%.(P<0.05). and the difference has statistical significance, so the depression rate of patients with insulin resistance is higher than that of patients without insulin resistance;2. The non-depression rate of light insulin resistance group is77.8%, the light-medium depression rate is22.2%, which are higher than58.6%and33.3%of severe insulin resistance group separately. That is, severe insulin resistance group has a higher occurrence rate of depression compared with light insulin resistance.3. The occurrence rate of light-severe depression of HBA1C>7.5%poor blood glucose control group is46.05%, and the occurrence rate of the non-depression is53.9%; the light severe occurrence rate of HBA1C<7.5%blood glucose control group is19.6%, and the non-depression rate is80.4%, so the better the blood glucose is controlled, the lower the occurrence rate of depression will be.4. Among diabetes patients with different depression degree, with the increase of depression degree, the diabetes life quality score is higher, the life quality score of non-depression patients is85.49±14.32; the light-medium depression patients is97.05±11.45; the severe depression patients is120.86±8.05; with the increase of depression degree, the life satisfying rate of patients is lower.5. The multiple-factor analysis of relevant factors that affect depression, logistic regression entering into equation is education degree, TC,ISI, BMI, HBA1C (%), diabetes life quality score and the past score, the results indicate that BMI, medical history score, life quality score has statistical significance, explaining that with the increase of BMI, medical history score and life quality score.decrease of ISI.the depression degree is more serious.[Conclusion]1.Depression in patients with insulin resistance and type2diabetes are related,and blood glucose control is better, the incidence rate of depression is lower;2.BMI, quality of life score,ISI score, history is an important influence factors of depression in patients with type2diabetes mellitus;3weight loss, control ofconcomitant diseases, in the treatment of depression and proper mental intervention may have help to reduce insulin resistance of diabetic patients with depression incidence.
