The Study Of The Relevance Between Tinnitus And Spleen-stomach

ObjectiveTinnitus is one of clinical common persistent ailments.So far, there is no big breakthrough in the research of the pathogenesis of tinnitus. Through collecting detailed tinnitus information and investigating the function of spleen-stomach of200patients with tinnitus, we will get the relationship between the onset of tinnitus and spleen-stomach, and learn more about the correlation of tinnitus severity and tinnitus characteristics and spleen-stomach function. Try to provide a reasonable selection for clinical treatment through investigating the etiology and pathogenesis of tinnitus from the perspective of TCM spleen-stomach.MethodsMake a survey among out-patients and inpatients with tinnitus who were treated in ENT Department of the First Affiliated Hospital of Guangzhou University of TCM from January2013to January2014. First, investigate the basic information of patients with tinnitus in detail which including tinnitus positions, sound, course, hearing condition, treatment, effect and so on. The conditions of the patients, history of spleen-stomach system diseases and living habit which is comprised of diet, medication, stress, sleep and so on, need to be inspected at the same time. Evaluate the tinnitus severity and function of spleen-stomach separately. Take tinnitus psychoacoustics tests and audiological examination. Then input all patient information into computer and establish database. At last, count the proportion of tinnitus patients who were existed spleen-stomach system diseases. Take an analysis of the different function of spleen-stomach between tinnitus patients and healthy adults. Compare the function of spleen-stomach between tinnitus patients who have different severity. Explore the potential factors of spleen-stomach dysfunction. Take a single factor analysis of spleen-stomach function with different characteristics of tinnitus patients to explore the relationship between tinnitus characteristic and the function of spleen-stomach. The significance of tinnitus characteristics to TCM syndrome differentiation are further analyzed.Results1. General information of tinnitus patients:There are200patients with tinnitus in total. Among them, the male are of121cases (60.50%), female are of79cases (39.50%). Their age ranges from15to79, and the average is42.68±12.99years old. The course of their diseases ranges from8days to33years, and the average is36.76±52.35months. As to career,57cases of professional and technical personnel,33cases of civilian,24cases of retirement,15cases of self-employed,11cases of civil servants,9cases of workers and8cases of students, other are43cases. About the occurred parts of tinnitus:39cases (19.50%) with the left ear,47cases (23.50%) with the right ear,95cases (47.50%) with two ears,12cases (6.00%) with head in,3cases (1.50%)with head out,3cases (1.50%) with the body outside, there is1case unable to explain clearly (0.50%). There are111cases (55.50%)with hearing loss,89cases (49.50%) with normal hearing.2. Among200cases of tinnitus patients, there are105cases (52.50%) who were existing spleen-stomach system organic disease. Compared with healthy adults, the spleen-stomach function average integral increases significantly (P<0.05).The spleen-stomach function integration of different grades of tinnitus increases with the aggravation of tinnitus.These differences are significant in statistics (P<0.05).3. The cause of spleen-stomach disorders survey:79.5%of patients like greasy and strong flavor,79.5%of them like cold food,66.50%of the them have pressure and80.00%of the patients have sleep problems before the occurrence of tinnitus.4. Take a single factor analysis of spleen-stomach function with people who have different tinnitus position, sound, tone sound frequency, residual inhibition test results and masking curve type respectively. These differences are not statistically significant (P>0.05).5. The inefficient treatment accounts84.02%among169patients with tinnitus who were treated by western medicine,kidney and promoting blood circulation to remove blood stasis of TCM.Conclusion 1. Tinnitus patients mostly have spleen-stomach system implement qualitative sex diseases and spleen-stomach dysfunction. The severity of tinnitus were negatively correlated with the spleen-stomach function roughly. The basic pathogenesis of tinnitus may be spleen-stomach dysfunction.2. Most of tinnitus patients usually have problems with unreasonable diet which including plentiful fat GanHou flavour and cold food, as well as stay up late or insomnia problems and pressure. They are the main causes of spleen-stomach dysfunction. Using cold medicines may become a cause of tinnitus.3.Tinnitus position, sound, tone sound frequency, residual inhibition test results and masking curve type have nothing to do with the function of spleen-stomach. And tinnitus characteristics which described by patients and obtained through tinnitus psychoacoustics test do not make any sense for TCM syndrome differentiation, either.
