Objective1. To evaluate the mitomycin-C (MMC) concentrations in rabbit aqueous humor, vitreous body、blood of vein after application during complex trabeculectomy by High performance liquid chromatography (HPLC) analyzer.2. Compare the difference of MMC concentrations in aqueous humor,vitreous body、blood of vein after application during complex trabeculectomy.3. Investigate the safe and effective concentrations of MMC in application during complex trabeculectomy.MethodsNinety New Zealand white rabbits were enrolled in this study. All the rabbits underwent complex trabeculectomy with application of mitomycin-C. During the surgery, a sponge soaked with MMC (0.2mg/ml,120seconds) was placed. Group A:a sponge soaked with MMC (0.2mg/ml,120seconds) was placed under the conjunctival flaps in forty-five New Zealand white rabbits(forty-five eyes) by random selection during complex trabeculectomy; Group B:a sponge soaked with MMC (0.2mg/ml,120seconds) was placed under the scleral flaps in forty-five New Zealand white rabbits(forty-five eyes) by random selection during complex trabeculectomy. The rabbit was underwent complex trabeculectomy with application of mitomycin-C only one eye as the experimental group by random selection, the other was none as the control group. Every experimental group was underwent complex trabeculectomy with application of mitomycin-C, the positions of MMC were irrigation with Ringer lactate solution80ml immediately. Then, aqueous humor samples, vitreous body samples0.1ml and blood of vein samples3ml were got with a needle by paracentesis in five rabbits five eyes after irrigation at5min、30min and60min respectively. The concentration of MMC was analyzed by High-Performance Liquid Chromatography (HPLC);The aqueous humor samples, vitreous body samples0.1ml and blood of vein samples3ml were got with a needle by paracentesis in6eyes of the control group by random selection before operation.Results1. The concentrations of MMC in aqueous humor was14.95±4.26ng/ml,27.26±6.33ng/ml,2.99±0.79ng/ml(group A)and44.81±8.42ng/ml,74.52±6.27ng/ml,17.89±6.85ng/ml (group B) at5min、30min and60min after the application of MMC respectively.2. The concentrations of MMC in vitreous body was2.30±0.76ng/ml,0.16±0.12ng/ml,0.13±0.09ng/ml (group A) and3.57±0.93ng/ml,1.21±0.81ng/ml,0.20+0.18ng/ml (group B) at5min、30min and60min after the application of MMC respectively.3. The concentration of MMC in blood of veins was35.32±17.26ng/ml,13.69±4.27ng/ml,0.33±0.55ng/ml (group A) and43.31±12.96ng/ml,25.32±5.85ng/ml,4.53±3.06ng/ml (group B) at5min、30min and60min after the application of MMC respectively.4. The peak aqueous humor concentrations were27.26±6.33ng/ml,2.30±0.76ng/ml,35.32±17.26ng/ml (group A),74.52±6.27ng/ml,3.57±0.93ng/ml,43.31±12.96ng/ml (group B) respectively.5. The peak concentrations of aqueous humor occurred at30min, and at5min both in vitreous body and blood of veins after application.6. The peak aqueous humor concentrations were0in all control group.Conclusions1. The peak aqueous humor MMC concentrations occurred at30min after the topical MMC application.2. The peak MMC concentrations of vitreous body and blood of veins occurred at5min after the topical MMC application.3. The MMC concentrations was higher in aqueous humor, in vitreous body, and in blood of veins under-scleral flap application group than that of under-conjunctival flap group.4. The concentrations of MMC were lower than the limits leading the organization damaging in rabbits eyes and blood of veins after complex trabeculectomy with application of mitomycin-C.
