[Objective]:Patients with hypertensive disorders in pregnancy are varying degrees of renal impairment, this study neutrophil gelatinase-associated lipocalin (NGAL), and serum cystatin C (Cys-C) of hypertensive disorders in pregnancy kidney damage early-specific expression, through joint determination of pregnancy-induced hypertension (PIH) in patients with blood neutrophil gelatinase-associated lipocalin (NGAL), and serum cystatin C (Cys-C), evaluation of NGAL and Cys-C in the forecast the diagnostic value of gestational hypertension kidney damage.[Methods]:1, medical options:Select February2013-August2013at the Second Hospital of Tianjin Medical University admitted obstetric diagnosis of pregnancy hypertension syndrome (PIH group)80cases of pregnant women, according to the diagnostic criteria,"Obstetrics and Gynecology" section Seventh Edition, is divided into four sub-groups, subgroups of gestational hypertension, mild preeclampsia subgroup, the subgroup of severe pre-eclampsia, eclampsia subgroups. The normal control group,40cases were hospitalized over the same period a normal pregnancy outcome in pregnant women. Both groups except chronic history of hypertension, diabetes mellitus, history of kidney disease and autoimmune diseases.2, the two groups were in the hospitalized within24h and72h post-natal blood, serum NGAL, serum Cys-C and blood Cr expression levels, analyze its relationship with the disease and its severity.3, respectively, paired t test, univariate ANOVA doing statistical processing, correlation analysis done using Spearman rho analysis.[Results]:1, the subjects admitted within24h and72h post-partum result of the comparison: NGAL concentration within24h72h admission test results compared with postpartum was significantly higher, the difference was statistically significant (P<0.05), Cys-C concentrations on admission compared with postpartum within72h24h the test results were significantly higher, the difference was statistically significant (P<0.05), Cr concentration hospitalized within24h72h compared with postpartum test results significantly increased, and the difference was significant (P<0.05).2, PIH group admitted within24h NGAL test results compared with the normal control group were significantly higher, the difference was statistically significant (P <0.05), PIH group admission test results within24h Cys-C compared with normal control group were significantly higher, the difference there was statistically significant (P<0.05), PIH group postpartum72h NGAL test results and normal control group, the difference was not strong, was not statistically significant (P>0.05), PIH group postpartum72h Cys-C testing results and the normal control group, the difference was not strong, not statistically significant (P>0.05).3, PIH group among the sub-groups found that with disease progression, pregnancy-induced hypertension subgroup, the subgroup with mild pre-eclampsia, severe pre-eclampsia, eclampsia subgroup, among subgroups single factor pairwise comparison, blood NGAL expression levels were increased, the differences between the groups were statistically significant (p<0.05); between Cys-C each group only gestational hypertension subgroup and preeclampsia subgroup there was a significant difference (p<0.05), The remaining subgroups were not statistically different (p>0.05).4, The subjects admitted within24h NGAL and correlation analysis showed that Cr and Cr NGAL was positively correlated0.905Subjects admitted within24h Cys-C and Cr correlation analysis showed that NGAL and Cr were positively correlated0.6635, For NGAL, Cys-C and Cr three indicators do ROC curves showed that the area under the curve of Cr0.706, the area under the curve of Cys-C0.812, NGAL area under the curve0.835, indicating that NGAL hypertensive disorders of pregnancy diagnosis of kidney damage highest value, followed by Cys-C, Cr diagnostic value minimum.[Conclusion]:1, The blood in patients with hypertensive disorders in pregnancy expression in both NGAL and Cys-C, and postnatal expression decreased Compared with normal control group of hypertensive disorders in pregnancy were significantly elevated blood NGAL and Cys-C levels were. Description NGAL and Cys-C levels are highly correlated with gestational hypertensive disorders of kidney damage.2, The expression of severity of pregnancy induced hypertension blood NGAL and Cys-C has difference, of NGAL and Cys-C levels, with the increase of serum Cr increases, further analysis showed that NGAL and Cys-C content severity of the disease is highly correlated.3, NGAL in the blood of patients with the same Cr content has a positive correlation, indicating NGAL levels of a high degree of correlation with severity of the disease; Cys-C content and Cr content is also a positive correlation, between serum NGAL and the severity of disease correlation coefficient is0.905(p=0.000), Cys-C and disease severity correlation coefficient was0.663(p=0.000). blood NGAL is greater than the correlation of Cys-C, blood NGAL more effectively than the Cys-C reaction severity of the disease.4, ROC curve results show that NGAL and Cys-C in predicting hypertensive disorders of kidney damage in pregnancy is higher than the value of the blood Cr, NGAL diagnostic highest value for hypertensive disorders in pregnancy kidney damage.In summary, blood NGAL and Cys-C can be used as an important predictor of gestational hypertension and renal injury severity.
