Association Between FECH IVS3-48 Mutation,KIM-1,NGAL And Liver And Kidney Injury Caused By Exercise In High Temperature And Humidity Environment

Part One:Correlation between FECH IVS3-48 gene mutation and heat stress injury in Chinese Han youthObjective:To study the correlation between IVS3-48T/C polymorphisms of FECH gene and multiple organ dysfunction caused by exercise with high temperature in young Chinese Han population.Methods: 141 healthy young men of Han nationality were randomly selected as the subjects in research.In high temperature and humidity environment,they were requested to finish a 3km running.The subjects were phlebotomized before and after the exercise,then the blood samples were used to detect the lactic acid,liver function(AST,ALT),kidney function(CCr)and cytoenzymology(CK,LDH)by an automatic biochemical analyzer.The Sanger sequencing was used to detect the FECH IVS3-48 alleles and genotypes of the subjects.According to the situation of tissue injury in subjects,they were divided into tissue injury group and non-tissue injury group.So we can analyze the correlation between IVS3-48T/C polymorphisms of FECH gene and sports injury with high temperature and humidity.Results:(1)Tissue damage: 32 cases(22.70%)of the subjects experienced tissue injury after exercise;(2)FECH IVS3-48 allele:the frequency of FECH IVS3-48T>C allele in this research was 34.04%;the injury group was 34.38%,and 33.94% the non-injury group;⑶FECH IVS3-48 genotype: the distribution of the three genotypes in the study was 62 cases(43.97%)with TT genotype,62 cases(43.97%)with TC genotype and 17 cases(12.06%)with CC genotype.(4)The correlation between genotype and sports injury: TT,TC,CC were12 cases(37.50%),18 cases(56.25%),2 cases(6.25%)in the injury group,50 cases(45.87%),44 cases(40.37%),15 cases(13.76%)in the non-injury group.There was no statistically significant difference in genotype distribution between the two groups(Fisher=2.709,p=0.273);⑸ Logistic regression analysis: There was no statistically significant difference in genotype distribution between FECH IVS3-48 genotype and tissue injury after excluding the influence of BMI and age.Conclusion: ⑴Strenuous exercise under high temperature and humidity environmen is a high-risk factor for tissue injury in human body,which can cause tissue injury in some young men;⑵FECH gene is polymorphic.The frequency of the IVS3-48T>C allele in this study was 34.04%;(3)The results of the study showed that the polymorphism of the IVS3-48T>C was not significantly associated with the risk of tissue injury caused by exercise with high temperature and humidity in Chinese young men.Part Two:The role of new biomarkers on early liver and kidney injury caused by exercise in high temperature and humidity environment and its molecular mechanism researchObjective: To explore new molecular biomarkers for human tissue injury caused by exercise in high temperature and humidity environment.Methods:100 soldiers who were received thermal acclimation training for more than one year took part in the research.They were divided into two groups: normal temperature and humidity group(ambient temperature 26±2℃,humidity 50±5%,and high temperature and humidity group(ambient temperature 33±2℃ and humidity 65±5%).Their blood samples were collected before and after 3km exercise,and the levels of serum ALT,ALT,Cr,NGAL and KIM-1 were detected by automated biochemical analyzers or ELISA.Results: ⑴The influence of temperature and humidity on sports injuries:The subjects in high temperature and humidity group are more likely to have liver and kidney damage than the normal temperature and humidity group.The increase in ALT,AST and Cr after exercise is statistically significant(p<0.001).We can observe the greater increase in high temperature and humidity group.For the increase of AST and Cr,the difference between high temperature and humidity group and normal temperature and humidity group was statistically significant(p<0.05).⑵Early molecular biomarkers changes in sports injuries:NGAL and KIM-1 increased significantly after exercise only in high temperature and humidity group(p<0.05),but the difference between two groups was not statistically significant(p> 0.05));⑶Multiple linear regression analysis of AKI:The temperature of exercise environment,running speed,ΔCCr have a linear relationship with ΔSCr,and there is no significant linear relationship with age,enlisted time,BMI,ΔNGAL and ΔKIM-1.Conclusion:⑴Exercise under high temperature and humidity environment is more likely to have liver and kidney injury than normal temperature and humidity environment;and the faster the speed,the greater the risk of liver and kidney injury is;⑵The early injury biomarkers KIM-1 and NGAL immediately detected after exercise have no obvious clinical significance for diagosing the liver and kidney injury.
