Objective: To explore the method and curative effect of the combined levator resection with tarsus resection to treat moderate or severe ptosis. Materials and Methods: This study included13patients (17eyelids, from Jul.2012to Feb.2014) with moderate or severe ptosis who underwent combined excision of the levator muscle and the tarsus. Of which4males and9females; four cases of unilateral, bilateral nine cases. Ages4to76years, with an average age of22years old. Postoperative follow-up of3months to12months, using the following indicators to examine, evaluate and analyze surgery:⑴position of upper lid rim⑵arc of upper palpebral rim⑶upper eyelid margin high degree of symmetry⑷upper eyelid closed degree⑸double fold crease⑹phenomenon of lagophthalmos in down-gaze⑺mobility of frontalis muscle⑻patient satisfaction⑼exposure keratitis.Results: The satisfying correction was achieved in all13patient cases (17eyelids) during the following time for at least3months to12months. The hypophasis existed for the earlier time after operation which recovered after3-6months postoperatively. Except one case (one eyelid) can not be completely closed in the force state, most of the eyelids can close naturally. Including two patients (two eyelids) overcorrection, there is one case (one eyelid) can be clearly observed hysteresis upper eyelid, the rest obtained good results. One patient with recurrence, eyelid folds shallow,disappear, and the rest were obvious. Does not appear exposure keratitis, and the curvature of the eyelid are satisfactory.Conclusion: The described technique appears to be effective for bothmoderate and severe ptosis, with better biomechanics and a satisfying aesthetic outcome. To achieve both conforms to the eyelid physiological functions, and improve the aesthetic effect, is satisfied operation for the treatment of the disease.
