ObjectiveTo explore the application of modified levator palpebral muscle shortening in correction of mild and moderate ptosis.MethodsFrom December 2016 to December 2017,86 patients with mild to moderate blepharoptosis were treated with levator muscle shortening.There were 46 patients with mild blepharoptosis(group A1)and 40 patients with moderate blepharoptosis(group B1).There were 33 patients with unilateral blepharoptosis and 11 patients with inconsistent degree of bilateral blepharoptosis(group A2),42 patients with the same degree of bilateral droop(group B2).Two rows of 6 points are needed to mark the sutures to be folded with nail purple during the operation.The upper row is exposed to the upper edge of the aponeurosis of the levator muscle of the upper eyelid after pulling open,and the distance between the lower row and the upper row is the shortening amount.It is according to the severity of blepharoptosis.Usually,for each correction of 1mm droop,the levator tendon membrane of the upper eyelid of 4-6mm needs to be shortened.A small incision was cut transversely at the label of levator tendon membrane of upper eyelid,and then slightly bluntly separate up to about 3mm.Using 7 ? 0 nylon line,through the upper lip of the upper incision and the lower lip of the lower incision,tighting knot,so that the two sutures could shorten the levator muscle of the upper eyelid.ResultsOf the 86 patients,there were no corrective over-operative patients,3 patients had insufficient correction,accounting for 3.49%;6 were basically satisfactory,accounting for 6.98%;77 patients were satisfied,accounting for 89.53%.In group A1,2 cases(4.35%)were insufficient correction,3 cases(6.52%)were basically satisfactory,41 cases(89.13%)were satisfactory;in group B1,1 case(2.50%)was insufficient correction,3 cases(7.50%)were basically satisfactory,36 cases(90.00%)were satisfactory,the difference was not statistically significant(P> 0.05).In group A2,2 cases(4.55%)were insufficient correction,4 cases(9.09%)were basically satisfactory,38(86.36%)cases were satisfactory,in group B2,1 case(2.38%)was insufficient correction,2 cases(4.76%)were basically satisfactory,39(92.86%)cases were satisfactory,the difference was not statistically significant(P> 0.05).ConclusionThe modified levator muscle shortening was used to correct mild and moderate blepharoptosis.There was no significant difference in the treatment effect.The operation was simplicity,the effect was lasting and stable,the trauma and risk were small,and it was convenient to adjust the postoperative effect again.It is worth popularizing and applying.
