Objectives To investigate the effectiveness and safety of conjoint fascial sheath(CFS)suspension combined with levator muscle of upper eyelid shortening and CFS suspension combined with blepharectomy in clinical application by comparing these two kinds of surgical methods which correct adult severe congenital ptosis in preoperative preparation,surgical operation,postoperative follow-up results and postoperative complications.Methods A total of 66 adult patients with severe congenital ptosis,aged 19-47 years,who were admitted to Hefei Hospital Affiliated Anhui Medical University from January2017 to May 2020 were selected,including 30 male patients,36 female patients,28 unilateral cases and 38 bilateral cases,with a total of 104 eyes.Using Patients were randomly divided into two groups using random number table method(n=33): the group of CFS suspension combined with upper eyelid shortening(Group A)and the group of CFS suspension combined with blepharectomy(Group B).In group A,ptosis was corrected by CFS suspension combined with upper eyelid shortening intraoperatively and in group B,CFS suspension combined with partial blepharectomy was used intraoperatively.Postoperative complications were treated timely and followed up for 6months,preoperative ptosis,correction of ptosis 6 months after surgery and postoperative complications were compared and observed.Results1.Comparison of general clinical data between the two groups: there was no significant difference between the two groups(P>0.05);2.The correction of ptosis in the two groups was followed up 6 months after surgery:group A was better than group B(P<0.05);3.Comparison of satisfaction rate of upper eyelid radian between the two groups 6months after surgery: group A was better than group B(P<0.05);4.Comparison of double emphasis on eyelid symmetry between the two groups 6months after surgery: there was no significant difference between the two groups(P>0.05);5.Comparison of related complications between the two groups 6 months after surgery:comparison of conjunctival prolapse: group B was significantly less than group A(P<0.05);comparison of other complications: there was no significant difference between the two groups(P>0.05).Conclusion Both of CFS suspension combined with levator muscle of upper eyelid shortening and CFS suspension combined with blepharectomy can correct severe congenital ptosis in adults effectively.CFS suspension combined with levator muscle of upper eyelid shortening is more advantageous in terms of long-term surgical outcomes,and there were no severe complications with neither surgical methods.Therefore,CFS suspension combined with levator muscle of upper eyelid shortening is more recommended...
