| Objective:Through the study of the fundamental data collection of patients with ulcerative colitis,identification of the TCM syndrome type and the questionnaire survey of TCM constitution types,we are trying to explore the characteristics of the incidence of ulcerative colitis,the correlation between the constitution and TCM syndrome type,in order to guide clinical practice.Methods:According to analyzethe literatures which I collected,and thereference to relevant standards,drawingup general data tables and constitutiontables to UC patients,on the basis of strict included and excluded criteria,I have collected100cases of eligible patients who had experiencedattacks of UC,who of them come from many hospitals in both Beijing and Shanghai from June2013to February2014.According to the dialectical classification and the judgment method of the constitution, I use SPSS17.0statistical software for data analysis,coming to the conclusion that the characteristics of UC and regular pattern of TCM syndrome types and theconstitution,and then discusse their relationship and correlation.Results(1)Of100patients with UC,the male to female ratio is1.174:l;The minimum age is27years old, the maximum is82years old,and the average age is46years old(Mean:46.13).There are three peakage of incidence, namely,32,37, and59years old.(2) We can not consider that permanent residence is related with gender and age of incidence.(p=0.617; p=0.620)(3) We can not consider that marital status and occupation are related in the incidence of UC (p=0.387).Be married (69%)> Divorced or widowed (22%)> Unmarried (9%). Mental workers (37%)> Be unemployed (31%)>The others (21%)> Manual workers (11%).(4) We can not consider that there are relationships among gender, occupation, marital status and age of incidence (p>0.05)(5)In the aggregate,100patients with UC are gathered, without past medical history of64cases,past medical history with hypertensionof19cases,withDiabetes of10cases and the ohers of7cases.(6)In terms of special fondness for some food,people who do havethis special fondness accounting for51%, of whomlove fatty and taste sweeter accounting for15%, then the pickled, spicy and acid or sweet.(7)In the course of disease,the leasttime is for one year, the most time is for51years, and average duration is for4.53years.The course tendto from one to five years.(8)Many cases invade and attack the rectum and sigmoid colon, accounting for49cases, followed by left colon, accounting for18cases,14cases of rectum and12cases of the whole colon,butmassive colon is least,only7cases.(9) Disease activity in young age group is more common, in catabasis for the elderly group. (10)There is close relationship among the course,the severity and lesion scope (P≤0.01),namely,course from one to five years,mainly the mild;from six to ten years,mainly the moderate;more than10years,the severe.The course of five years or less,it occurs like the rectum, sigmoid colon (55.1%), left colon (19.2%),rectum(14.1%),massive colon (6.4%),the whole colon (5.1%). The course between five years and ten,the incidence of the whole colon is37.5%,left colon and rectum18.8%,rectum and sigmoid colon the same as massive colon12.5%;More than10years, the incidence of rectum and sigmoid colon is66.7%,the whole colon33.3%.(11) We think the distribution of TCM syndrome type has significant meaning (p=0.000<0.01).For example,large intestine damp heats syndrome(38%)> wet aggregates spleen deficient syndrome (28%)> liver penting up and spleen deficiency syndrome(16%)> deficiency of spleen and kidney Yang syndrome (9%) simultaneous occurrence of cold and heat syndrome (5%)> Yin anaemia deficiency syndrome (4%). Large intestine damp heats syndrome and wet aggregates spleen deficient are the main TCM syndrome type of UC.(12)Distribution of the constitution types in UC:Biased constitution types in UC account for89%,damp-heat type (30%)> qi-deficiency type (21%)> qi-depression type (16%)> gentleness type (11%)> phlegm-wet type (10%)>special inherited type (5%)> yang-deficiency type=Yin-deficiency type (3%)> blood-stasis type (1%)。(13)In the relationship of TCM syndrome type and constitution types in UC(p=.000<0.01), gentleness type are closely related to wet aggregates spleen deficient syndrome;In biased constitution types,the largest proportion of qi-deficiency type is wet aggregates spleen deficient syndrome (61.9%),the largest proportion of yang-deficiency type is deficiency of spleen and kidney Yang syndrome; the largest proportion of Yin-deficiency type is wet aggregates spleen deficient syndrome,Yin anaemia deficiency syndrome, simultaneous occurrence of cold and heat syndrome,accounting for33.3%equally;the largest proportion of phlegm-wet type is large intestine damp heats syndrome;damp-heat type lay the absolute position in large intestine damp heats syndrome, accounting for93.3%;the largest proportion of blood-stasis type is liver penting up and spleen deficiency syndrome;the ratio of liver penting up and spleen deficiency syndrome is more than half in qi-depression type, accounting for62,5%;the largest proportion of special inherited type syndrome is Yin anaemia deficiency syndrome.(14)There is relationship among the constitution and occupation,the course,the severity,lesion scope (p<0.05).Most of the patients with UC are mental workers. The majority of the unemployed is damp-heat type, and the majority of the manual worker is blood-stasis type,whereas qi-deficiency and gentleness type are for other groups.The patients who suffer from UC about one to five years, are mainly gentleness type, qi-deficiency type, Yin-deficiency type, yang-deficiency type, phlegm-wet type, damp-heat type, blood stasis type and special inherited type.While the course about5-10years of UC patients, are mainly qi-depression type.There are some types more closely related to the mild in UC, qi-deficiency type, yang-deficiency type, yin-deficiency type, phlegm-wet type, damp-heat type and special inherited type.Blood stasis type is given priority to the moderate, qi-depression type to the severe. Incidence of gentleness type in UC is all located in rectum; Incidence of qi-deficiency type, yang-deficiency type,yin-deficiency, damp-heat type in UC is mainly located in the rectum and sigmoid colon Incidence of special inherited type in UC is mainly located the left colon;Incidence of phlegm-wet type is in massive colon;And blood-stasis type and qi-depression type are mainly throughout the whole colon.(15)There is closely clinical correlation between TCM syndrome type and severity of UC (p=0.002<0.05), Of them,the mild mainly is for large intestine damp heats syndrome, wet aggregates spleen deficient syndrome, deficiency of spleen and kidney Yang syndrome,and Yin anaemia deficiency syndrome; The moderate is for simultaneous occurrence of cold and heat syndrome;The severe is mainly for liver penting up and spleen deficiency syndrome.(16) We can not consider that the constitution is related to gender, age of incidence and marital status (p>0.05)ConclusionsThere is significant correlation between the constitution and TCM syndrome type in UC.Constitution types may decide or turn into TCM syndrome type of UC,and have this nature of the susceptibility and tendencies to TCM syndrome type.This theory offers a new way for the treatment of UC, opens up a wider perspective:Using the combination between the constitution and the dialectical classification of the two complement each other, We can not only undertake mission in sub-health crowd, correct its biased constitution early;but also for UC treatment of patients with UC,combine pathological feature and TCM syndrome typewith the constitution,overally review, comprehensive angles to explore the occurrence,development and mutation of UC.Combining "disease" and "syndrome " with" Constitution "together, It not only fully embody the soul and the kernel of the holistic concept of TCM, but also embody the essence of TCM individualized diagnosis and treatment,and provide objective basis for diagnosis and treatment of traditional Chinese medicine, in order to achieve "Prevention before disease onset,preventing disease from exacerbating".Do the best doctor for prevention before disease onset really. |
