In the acupuncture clinic, when the pathological changes of certain part of the body occur needing to be treated by acupuncture, the performer must first clear and definite the laws of meridian traveling courses of the lesion, which is basic theory of identifying the meridians which the pathological changes refer to. Secondly, the performer should have knowledge of the meridians that the relevant diseases mostly refer to, which can help to scope the meridians to be differentiated. Finally, the performer should clear how to differentiate the meridians related to diseases. So it is necessary to discuss how to examine and judge the meridians which the pathological changes refer to, including ways and parts of body using for examination. In order to clear and definite the parts of body using for examination, it’s necessary to study the main distribution parts of the acupoints used to treat relevant diseases. The both are the same, because the acupoints have examination effect as well as therapeutical effect. Meridian examination, examining the meridians related to relevant diseases, can discover the pathological changes of the meridians, identify the meridians which pathological changes refer to and guide acupuncture treatment.This paper mainly included two parts:The one was to study the law of the meridian systems circulating around ear and surrounding tissue. The other was to study the examination regularities of common diseases of ear in acupuncture clinic, including the meridians which relevant diseases commonly refer to, common examination parts of body and pinpointed examination sites.MethodsThe author collected all of the meridian system components associated with ears from ancient acupuncture literature (Huangdi’s Canon of Medicine mainly), including the line-related and the disease-related, to study their holistic regularity.Then this paper discussed the meridian examination law of ears from the point of view of specific diseases. First, it was necessary to identify common diseases of ears which acupuncture treatment is suitable for. And then ancient classic literature of acupuncture, later acupuncture literature and modern acupuncture literature were respectively retrieved according to the various common diseases of ears. Then to collect the acupoints used to treat the various common diseases of ears in the literature of each period and sum up their times (the frequency of occurrence of the acupoints) to be used, and to analyze the distribution characteristics of the frequency of occurrence of the relevant acupoints in body parts and the meridians. To respectively discuss the meridian examination law of the various common diseases of ears in the acupuncture clinic, including mainly related meridians (ie common meridians applied for examination), common body parts applied for examination and pinpointing the examination sites, by summarizing ancient evolution.Results1. The meridian systems circulating around ear and surrounding tissue In ancient acupuncture literature, the meridian system components associated with ears are:①Meridian System:Triple Energizer Meridian of Hand-Shaoyang, Small Intestine Meridian of Hand-Taiyang, Large Intestine Meridian of Hand-Yangming, Gallbladder Meridian of Foot-Shaoyang, Bladder Meridian of Foot-Taiyang, Stomach Meridian of Foot-Yangming, Kidney Meridian of Foot-Shaoyin, Liver Meridian of Foot-Jueyin, Lung Meridian of Hand-Taiyin;②endon System:Triple Energizer Meridian of Hand-Shaoyang Musculatures, Small Intestine Meridian of Hand-Taiyang Musculatures, Gallbladder Meridian of Foot-Shaoyang Musculatures, Bladder Meridian of Foot-Taiyang Musculatures, Stomach Meridian of Foot-Yangming Musculatures;③ollateral System:Large Intestine Meidian of Hand-Yangming collaterals, Stomach Meridian of Foot-Yangming collaterals, Lung Meridian of Hand-Taiyin collaterals, Spleen Meridian of Foot-Taiyin collaterals, Heart Meridian of Hand-Shaoyin collaterals, Kidney Meridian of Foot-Shaoyin collaterals;④Pericardium Meidian of Hand-Jueyin divergent branches;⑤Yang Link Vessel.2. Literature retrieval results of the common diseases of ears in the acupuncture clinicThe author referred to "The National Standard Database about Classification and Codes of TCM Diseases"which belongs to "TCM Terminology Standard Database", and "The National Standard Database of TCM Clinical Diagnosis Terminology (disease section)"was also referred to, found that tinnitus, deafness and otitis media are more common in the acupuncture clinic.In ancient classic literature of acupuncture, the author retrieved a total of115literature, which involves52literature about tinnitus,30acupoints and74points times; involves63literature about deafness,32acupoints and75points times; involves no literature about otitis media.In later acupuncture literature, the author retrieved a total of443literature, which involves166literature about tinnitus,50acupoints and228points times; involves250literature about deafness,75acupoints and352points times; involves27literature about otitis media,10acupoints and36points times.In modern acupuncture literature, the author retrieved a total of144literature, which involves56literature about tinnitus,63acupoints and309points times; involves82literature about deafness,56acupoints and424points times; involves6literature about otitis media,13acupoints and18points times.We could know the regularities of distribution of the acupoints related to various diseases, of the meridians and parts of body, by summarizing all of the acupoints, then to get the examination law of various diseases.Conclusion1. The author found that the number of yang meridians is more than that of yin meridians, and hand-san-yang meridians are closer to ears than foot-san-yang meridians by studying the law of the meridian systems circulating around ear and surrounding tissue. Generally speaking, yang meridians treat excess syndrome and heat syndrome, and yin metidians treat deficiency syndrome and cold syndrome, so the author thought that in the acupuncture clinic it is mostly excess syndrome of ears that acupuncture treatment is suitable for.2. Tinnitus, deafness and otitis media are primarily related to the yang meridians. Tinnitus and deafness are mainly related to Triple Energizer Meridian of Hand-Shaoy ang, Gallbladder Meridian of Foot-Shaoyang and Small Intestine Meridian of Hand-Tai yang. Otitis media is primarily related to Triple Energizer Meridian of Hand-Shaoyang, Gallbladder Meridian of Foot-Shaoyang and Stomach Meridian of Foot-Yangming. The common body parts used for examination of tinnitus are head and face, hands and fe et. The common body parts used for examination of deafness are head and face, hand s, feet and forearms. The common body parts used for examination of Otitis media ar e head and face. So in addition to the head and face, the other body parts applied fo r examination are mainly located below the elbow and knee. The pinpointed examinati on sites means the acupoints or line (distance between two acupoints), at correspondin-g examination-body-part of each examination-meridian of each relevant disease, often used for meridian examination. Referring to ancient and modern literature, the high-fre quency3acupoints (line included) at corresponding examination-body-part of the relev ant meridians of each disease were chosen for meridian examination generally.
