Based On The Literature Of Shoulder Meridians And The Clinical Examination Of Shoulder And Meridians In The Clinical Examination Of Shoulders

According to the acupuncture and moxibustion literature and my supervisor's rich clinical experience,Firstly I will summarize the running and crossing rules of the system of meridians and collaterals(including meridians,collaterals,divergent meridians,meridian sinew and their relations with diseases)in different parts of human body.Secondly,I will search the distribution of the system of meridians and collaterals in different parts of the human body,and its relationship with the anatomy,physiology and pathology.In addition,I will summarize the connection between the distribution of meridians and collaterals and pathological changes in certain parts of human body(taking scapulohumeral periarthritis as an example),along with the features of disease reaction and meridian diagnosis.I have completed the research in order to provide theoretical basis for clinical meridian treatment according to syndrome differentiation and to improve the pertinence and effectiveness of acupuncture and moxibustion therapy.The main body of my thesis consists of three parts.They are as the followings:The first part is the review of scapulohumeral periarthritis.On the basis of the current treatment situation and the knowledge of scapulohumeral periarthritis in Chinese medicine and modern medicine,I will introduce the disease's cause,the interpretation of the cause and its various treatments.Chinese medicine,based on the concept of the whole,analyzes the disease's cause and the interpretation of the cause from the point of treatments according to syndrome differentiation.Despite various treatments including acupuncture and moxibustion,Chinese herbs,massage,cupping and guiding,Chinese medicine develops its unique therapies of scapulohumeral periarthritis in an integrated approach.As for modern medicine,it focuses on analyzing the pathogenesis of shoulder periarthritis from the standpoint of the anatomy,physiology and pathology,which treats the disease with medical,physical therapies and rehabilitation training.The two kinds of therapies play an important role in medical practice,but they still have some shortcomings.The importance of the curative effect of meridians and collaterals examination and running in acupuncture therapy should be strengthened.The second part is about meridians and collaterals diagnosis and meridians and collaterals running in shoulder.I will give introductions to the ancient and modern literature about the rules of meridians and collaterals diagnosisand to the meridians and collaterals running and common diseases in the shoulder.Meridians and collaterals detection and diagnosis is an important part in clinical acupuncturetherapy,as well as a necessary step in four ways of diagnosis(look,listen,question and feel the pulse)in Chinese medicine.Especially the detection and diagnosis method of watching,pressing,feeling the pulse and so on in skin,meridians,collaterals and acupoints further our understanding toward the disease.Chinese medical treatment based on syndrome differentiation regards "The signals of a disease usually first appear in the skin." as a canon.Meridians and collaterals detection and diagnosis is a practice of this canon in clinic.Once you find the color or shape of the skin changes or feel abnormal,the analysis of the disease,its pathomechanism and prognosis should be practiced immediately.The third part is my analysis of the retrieved data.I will extract some ancient and modern examples of scapulohumeral periarthritis,and then summarize the principles of acupoints and meridians detection and diagnosis methods from the cases.Based on the analysis of the fluency of meridians and collaterals running in different parts of human body and in different acupoints,I will summarize the similarities and differences in ancient and modern medical therapies of scapulohumeral periarthritis to improve the effectiveness of treatment.ObjectiveIn this paper,the combination of literature and clinical perspective of two meridian system circulation,shoulder disease,acupoints on periarthritis of shoulder shoulder typical disease of meridian,hope that through this study provides ideas and methods for the diagnosis and treatment of clinical diagnosis and treatment of periarthritis of shoulder.MethodologyI have retrieved 22 classical Chinese medical literatures including the Huangdi's Canon of Medicine,the Canon on Medical Problems,the Huangdi's Mingtang Classic and so on,11 contemporary periodicals.I have analyzed the using frequency of acupoints in order to pick out the standard acupoints and summarize the therapy principles of scapulohumeral periarthritis.By comparing the ancient and modern therapies,I have concluded their similarities and differences to find out the accurate meridians and collaterals examination location of scapulohumeral periarthritis.ResultSelected and counted both by labor and computer,234 pieces of ancient works and 84 acupoints with 337 times,as well as 89 pieces of contemporary works and 54 acupoints with 575 times are found.Acupuncture researches in ancient and modern documents all mentioned some frequently-used meridians and collaterals,such as the small intestine meridian,the triple energizer meridian,the large intestine channel of hand-yangming and the gall bladder channel of foot-shaoyang.The frequently-used acupoints include Point Jianyu,Quchi,Jianliao,Zhigou,Zhongzhu,Hegu,Guanwai and Jianjing.ConclusionBased on the ancient and modern document retrieval I've mentioned above,in the treatment of scapulohumeral periarthritis,I come to the conclusion that the three yang meridians and the gall bladder channel of foot-shaoyang are important in both ancient and modern times.The lung channel of hand-taiyin and spleen channel of foot-taiyin are frequently used in modern medicine,but seldom mentioned in ancient medicine.The spleen channel of foot-taiyin and lung channel of hand-taiyin belong to the taiyin meridian,but they have different tasks.The foot-taiyin controls the organs while the hand-taiyin controls the skin.Therefore they are closely related.At the same time,scapulohumeral periarthritis usually is caused by wind-cold and insufficiency of vital energy and blood.As a result,more studies and researches about the two meridians should be made in clinical level.In one word,the principle of selecting acupoints to treat scapulohumeral periarthritis in acupuncture and moxibustion therapy is as the following:The therapy is mainly based on the three yang meridians of hand and partially based on the gall bladder channel of foot-shaoyang,the lung channel of hand-taiyin and spleen channel of foot-taiyin.The treatment method embodies the useful principle of "treat the parts of human body where meridians run"to select acupointsin acupuncture and moxibustion therapy.At the same time,it considers the relationship between exogenous pathogenic wind and qi-blood which lead to scapulohumeral periarthritis.Clinical classification is also taken into consideration.My thesis is aimed to make the clinical selection of acupoints more accurate and nominative,and provide theoretical basis for selection of acupoints to treat scapulohumeral periarthritis.
