Background:The knee osteoarthritis is a degenerative disease of the knee function, which is one of the leading causes of the lower quality of life for the elderly. Abdominal acupuncture found by professor Bo Zhi yun40years ago is a new kind of micro needle therapy,which regards the Shen Que regulation system as the core of the human. Abdominal acupuncture treatment on the knee osteoarthritis, acupuncturing according to the turtle diagram to locate the points, finding out response relationship points, has received a satisfactory curative effect after a large number of clinical application of traditional Chinese medicine; and compared with other methods of acupuncture and moxibustion, the most obvious feature of the abdominal acupuncture is painless, especially easy to be accepted for the patients sensitive to the pain. At present, the application of abdominal acupuncture has achieved dialectical streamline, standardization prescription, operation standardization, which has provided us the possibility of the clinical research and application.Objective:To compare the clinical efficacy of abdominal acupuncture and traditional acupuncture (body acupuncture) treatment of knee osteoarthritis,so as to get an objective evaluation of clinical efficacy of the treatment of abdominal acupuncture for osteoarthritis of the knee, at the same time, observing and analyzing the analgesic effect of abdominal acupuncture.Methods:We identified a total of96patients who attent the Acupuncture Department of Dong Zhimen Hospital during October2012to January2014. All the patients met our selection criteria, and the design project of experiment was adopted completely random. The patients were divided into three groups, and each group had32patients, but only91patients entered the stage of result analysis with5loss in the midway, results are as follows:30cases of Traditional acupuncture (body acupuncture) group,30cases of Abdominal acupuncture1group,31cases of Abdominal acupuncture2group. Points selection of Traditional acupuncture (body acupuncture) group:Xi Yan, Liang Qiu, Yang Ling quan, Xi Yang guan, Yin Ling quan, A Shi points. Points selection of Abdominal acupuncture1group:Zhong Wan, Guan Yuan, Hua Rou men (the affected side), wai ling (the affected side),qi pang (the normal side),Rheumatism point (the affected side), Da heng (double). Selection of Abdominal acupuncture2group:Zhong Wan, Shui Fen, Qi Hai, Guan Yuan, Guan Yuan xia, Wai Ling (the affected side), Rheumatism point(the affected side),Qi Pang (the normal side),Qi Xue(double). The acupuncturing bodily parts were illuminated30minutes by TDP therapeutic instrument. The treatment was done3times a week, and10times as a course. According to the HSS knee scale, efficacy and state of an illness were analysed. Clinical symptoms was analysed by all the criterias of the HSS knee scale. Comparison of pain degree of knee osteoarthritis was in accordance with the VAS knee scale by abdominal acupuncture in pretherapy and post-treatment.Results:The VAS score of three groups decreased in pretherapy and post-treatment, differences were statistically significant (P<0.05); On the comparison of three groups by criteria of the VAS after treatment, there were differences between the three groups (P<0.05) by the nonparametric test. Compared with traditional acupuncture (body acupuncture), both Abdominal acupuncture1group and Abdominal acupuncture2group had statistical significance (P<0.05), and both reduce more pain value than traditional acupuncture (body acupuncture); Compared with Abdominal acupuncture2group, Abdominal acupuncture1group has statistical significance (P<0.05) with less pain value than it.Comparison among three groups before treatment, the overall HSS score had no difference; The HSS score of three groups of HSS score were higher after treatment,compared with the pretherapy, differences were statistically signif icant(P<0.05). The HSS value of the three groups compared after treatment, there were differences between three groups (P<0.05) by the nonparametric test. Compared with traditional acupuncture (body acupuncture),both Abdominal acupuncture1group and Abdominal acupuncture2group had statistical significance (P<0.05),but there was no statistical significance between Abdominal acupuncture1group and Abdominal acupuncture2group. After treatment, the results evaluated by criteria of the HSS knee scale were as follows The three groups comparing with each other in pretherapy and post-treatment all had statistical significance (P<0.05) in relieving pains. Compared with traditional acupuncture (body acupuncture),the differences of both Abdominal acupuncture group and Abdominal acupuncture2group had statistical significance (P<0.05) with the comparison after treatment;Compared with Abdominal acupuncture1group, Abdominal acupuncture2group had statistical significance (P>0.05).The three groups comparing with each other in pretherapy and post-treatment all had statistical significance (P<0.05) in the improvement of joint function. The three groups comparing with each other in the post-treatment all had no statistical significance (P>0.05). The three groups comparing with each other in pretherapy and post-treatment all had no statistical significance (P>0.05), comparing with each other in pretherapy and post-treatment among the three groups in the improvement of quadriceps muscle power. The three groups comparing with each other in the post-treatment all had no statistical significance (P>0.05). The three groups comparing with each other in pretherapy and post-treatment all had no statistical significance (P>0.05) in the improvement of strengthening the joint stability;both Abdominal acupuncture1group and traditional acupuncture (body acupuncture)had statistical significance (P<0.05), comparing with each other in post-treatment among the three groups.After treatment, the results evaluated by criteria were as follows:Compared with traditional acupuncture(body acupuncture)and Abdominal acupuncture2group,Abdominal acupuncture1group had statistical significance,(P<0.05). Compared with traditional acupuncture (body acupuncture), the differences of both Abdominal acupuncture1group and Abdominal acupuncture2group had statistical significance (P<0.05) with the comparison on the changes of state of an illness; Compared with Abdominal acupuncture1group, Abdominal acupuncture2group had no statistical significance (P>0.05)Comparison of pain degree of knee osteoarthritis was on the basis of the VAS knee scale by abdominal acupuncture in pretherapy and post-treatment, the results were as follows:Abdominal acupuncture1group:compared with the first time before treatment,every time had significantly statistical differences before treatment (P<0.05),and the VAS score of three groups was decreasing in post-treatment; compared with the first time after treatment,the pain relief was obvious after the third treatment. Abdominal acupuncture2group:All had statistical significance (P<0.05), comparing with each other in post-treatment, incating that immediate analgesia effect of Abdominal acupuncture2group was remarkable.compared with the first time before treatment,every time had significantly statistical differences before treatment (P<0.05), incating that pain value showed a trend of gradual decline. All had statistical significance (P<0.05), comparing with the first time in post-treatment, incating pain was decreasing after treatment. The VAS pain score results of abdominal acupuncture1group and abdominal acupuncture2group after each treatment:compared with the first treatment among all the comparisons after treatment,the second post-treatment had no more statistically significant difference than the first one (P>0.05), while there was significantly statistical difference (P<0.05) after the third one; compared with the last treatment among all the post-treatments,the seventh and the eighth ones did not have statistically significant difference (P>0.05), however, the others did have. Since the seventh time,both of the groups had differences (P<0.05)Conclusions:1. Abdominal acupuncture and tbody acupuncture therapy both have a significant effect on the treatment of osteoarthritis of the knee.2. The abdominal acupuncture and traditional acupuncture (body acupuncture) both have prominent clinical efficacy on the knee osteoarthritis in the view of relieving pain and improving activities on knee function; however, the abdominal acupuncture has a better analgesic effect than the body acupuncture.3. In the view of relieving pain and shortening the beginning time of analgetic effect,the abdominal acupuncture1group is better than abdominal acupuncture2group. The prescription is as follows:Zhong Wan, Guan Yuan, Hua Rou men (the affected side),wai ling (the affected side), qi pang (the normal side), Rheumatism point (the affected side), Da heng (double).
