ObjectiveBy analyzing the correlation between chronic pain and pain catastrophizing, social support, attachment, depression, and building the chronic pain depressive attachment model, due to enrich the depression theoretical research in patients with chronic pain, and early detect depression in patients with chronic pain, provide psychological basis for rehabilitation.MethodsThis study involved a convenience sample of310patients with chronic pain, the subjects were from Chongqing Southwest Hospital. Adopting the questionnaires: Short-Form McGill Pain Questionnaire, Self-Rating Depression Scale, Chinese translation of the Pain Catastrophizing Scale,social support revalued scale, Experiences in close Relationship’s Inventory.Result(1) pain:the pain time of0-9years in patients are in the majority (64.21%), the patients are mostly intermittent chronic pain (69.57%); feeing pain intensity is more severe pain (30.11%); the present pain intensity is more pain discomfort (46.49%).(2) depression:the comorbidity rate of depression and chronic pain is32.00%;68.00%of the patients without depression, slight to mild depression rate isl5.66%, moderate to severe depression rate is11.67%, and4.67%of the patients are severe depression.The depression of patients with different genders (p=0.226) and different educations (p=0.225) have no significant difference; The depression of patients with different ages (p=0.041), different marital status (p=0.007), different works (p=0.001), different residences (p=0.000), different types of health insurance (p= 0.000) and different family economic status (p=0.000) have significant difference.(3) Attachment:Attachment avoidance of patients with different genders (p=0.014) and different depression extents (p=0.000) have significant difference, different ages (p=0.610) have no significant difference; attachment anxiety of patients with different genders (p=0.013), different ages (p=0.006), different depression extents (p=0.000) have significant differences.(4) The correlation analysis showed that each dimension of pain intensity and depression are significantly positive correlation (0.41,0.30,0.45,0.32); each dimension of attachment avoidance and pain intensity are positively correlated (0.15,0.03,0.17,0.04), attachment anxiety and each dimension of pain intensity are positively correlated (0.22,0.11,0.19,0.25); depression and attachment avoidance, attachment anxiety showed a significant positive correlation (0.34,0.41).(5) Regression analysis showed that the attachment was a moderator between chronic pain and depression (F=18.109, p=0.000); the attachment was a moderator between pain catastrophizing and depression (F=27.658, p=0.000); the attachment was a moderator between social support and depression (F=12.042, p=12.042).(6) SEM analysis showed pain catastrophizing, social support and attachment were mediation variables between chronic pain and depression. The model fitting indicators χ2/df=2.873, RMSEA=0.079, GFI=0.927, NFI=0.900, CFI=0.931.chronic pain through four paths depression:chronic painâ†'catastrophizingâ†'depression, chronic painâ†'attachmentâ†'depression, chronic painâ†'attachmentâ†'catastrophizingâ†'depression, chronic painâ†'attachmentâ†'social supportâ†'depression.Conclusion(1) The patients’pain time is generally longer and more intermittent pain, Patients have all sorts of degree of pain intensity, and present pain intensity is generally lower in hospitalized patients.(2) Patients have a higher incidence of depression, pain intensity, economic status, attachment have great relationship with the degree of depression.(3) The prediction of pain intensity, catastrophizing and social support on depression are subject to the attachment.(4) Chronic pain is an important factor in the prediction of depression in patients, it through the catastrophizing and attachment indirect predict depression; pain chronic through the attachment and catastrophizing indirect predict depression; pain chronic through the attachment and social support indirect predict depression.
