Development Of An Item Bank For The State-trait Depression Questionnaire And The Structural Equation Modeling

During depressive symptoms becomes increasingly important in science as well as medical practice. The diagnosis and treatment for depression are very urgent, but according to recent investigations, lots of patients with depression still choice to be managed and treated in primary care and the physician's diagnostic of patients' status are not precision. The survery found in China only 21% primary care doctors can diagnose the depression and only 5% of patients received the appropriate antidepressant treatment. Therefore, it is very necessary to improve the ability of recognition of psychological problems in non psychiatric health.This study summarized the common symptoms of the 9 major types of depression and development of an Item Bank for the State-Trait Depression Questionnaire with the 9 symptoms. The definition of the symptoms is consistent with the DSM-IV, the ICD-10 and the CCMD-3.Here are the 3 main results of this research:1. Classification of 9 types of symptoms according to state-trait depression. State depression include: psychomotor problem, physiological condition(somatic sensation, sleep condition, diet condition), depressive mood, aprosexia. Trait depression include: negative attitude, self-reproach, social relations, suicidal idea, anhedonia.2. The item pool exclude items with slope of <0.70 and the test information of symptom dimension reached the ideal position. The results of EFA show a good factor structure, all the factors loading above 0.40 in CFA, results were CFI & TLI > 0.95, RMSEA < 0.05, the estimated value of WRMR is also within the acceptable range. Cronbach alpha of the state and trait depression scale are 0.949 and 0.956, and the average value in each dimension of the state(trait) depression scale's Cronbach alpha was 0.768(0.790).3. SEM analysis results and shows that there are 5 state-trait depression symptom dimensions related model. Model 1: negative attitude on depressive mood & aprosexia, and the path coefficient are 0.804& 0.175. Model 2: self-reproach on depressive mood & aprosexia, and the path coefficient are 0.71 & 0.27. Model 3: social relations on depressive mood & sleep condition, and the path coefficient are 0.725 & 0.187. Model 4: suicidal idea on depressive mood, and the path coefficient is 0.87. Model 5: anhedonia on depressive mood, and the path coefficient is 0.95.The major conclusion of this study are:1. The state-trait depression questionnaire are valid and, reliable. It has good psychometric properties. It can be applied to detect the depression.2. The state-trait depression symptom dimensions related model will provide reference for future clinical diagnosis.
