Hospital Clinical Pathway Management Practice Research

Clinical pathway is a kind of medical model that based on patient diagnosis,intergrating multidisciplinary study and including care, medical examination andstandardized treatment process. Since National Health and Family PlanningCommission of the People’s Republic of China started clinical pathway pilot work,medical institutions at all levels across the country have actively responsed andachieved good results, effectively alleviated the problems of seeing a doctor difficultlyand expensively and over treatment, at the same time, well reduced mediacl cost andimproved medical quality. But during the practice process across the country this modelappeared many problems that functions of the government failed to effectively playing,legal responsibility of clinical pathway management’s relationship between doctors andpatients marked out unclear, hospital clinical pathway informatization level behaved notvery well and so on. This article based on public choice theory, standardized theory andPDCA cycle theory and the concepts of clinical pathway,DRGs,single-disease payment,medical quality and hospital clinical pathway management, combined with theoperation of A hospital in Suzhou, systematically analyzed our hospitals clinicalpathway practice achievements and deficiencies of our hospitals in recent years.Putfoward some suggestions,during the planning phase,hospital clinical pathwaymanagement practice should clear the functions of the government,define the legalresponsibility,build a management system,perfect the performance indicators. Duringthe execution phase,hospital clinical pathway management practice should ensure themedical safety,exert the supervision effect,combine the bed management, improve thedepartments coordination.During the check phase,hospital clinical pathwaymanagement practice should implement variation mangement,ensure the medicalquality,enhance the doctor-patient communication,strengthen the informationmanagement.During the feedback phase,hospital clinical pathway management practice should accelerate the reform of the health care,normalized the medical cost,proceed theeffect appraisal,intensify the personnel training.
