The Comparative Study Of Wide-necked Intracranial Aneurysms By Craniotomy And Intravascular Microsurgery

Objective:Explore the wide-necked aneurysms rupture ultra-earlytreatment methods, safety and prognosisMethods:Retrospective analysis of our department between January2010to December2013,the use of stent-assisted embolization or craniotomy clippingultra-early treatment (48h) wide-necked intracranial aneurysms rupture of50cases, aneurysm55,comparing two groups of Surgery-relatedcomplications,recurrence and prognosis.Grade of the degree of aneurysmocclusion with Raymond,adopt improved prognosis,mRS score0~2was dividedinto good,3~6are divided into bad.Result:With Stent-assisted embolization group,28of31patients withaneurysms,after a good rate(82.1%),Craniotomy clipping group of22patientswith a total of24aneurysms,after a good rate(81.8%).Groups in the prognosisand treatment was not statistically significant.Craniotomy clipping group thanthe Stent-assisted embolization group(P <0.05)in the postoperative occlusionrate.Craniotomy clipping stent-assisted embolization group compared with thehigh incidence of surgery-related complications, but there were no significant differences (P>0.05).Stent-assisted embolization higher recurrence rate thancraniotomy clipping group, but no significant difference between the twogroups;Craniotomy clipping group, the average length of stay compared withstent-assisted embolization leader, the two groups was statistically significant (P<0.05).Conclusion: Effective method of surgical clipping and Stent-assistedembolization craniotomy are wide-necked intracranial aneurysms. Surgicalclipping of wide-necked aneurysms densification rate than Stent-assistedembolization group. Stent-assisted embolization group of wide-neckedaneurysms higher recurrence rate than surgical clipping group, but no significantdifference between the two groups;Stent-assisted embolization group relative tothe group of surgical clipping procedure-related complication rate, less trauma,quicker recovery.
