BackgroundSince Australian scientists Barry J. Marshall and J. Robin Warren isolated Helicobacter pylori for the first time from the gastric mucosa biopsy tissue of patients with chronic active gastritis in1983, the study of Helicobacter pylori caused widespread concern. Early researches suggest that Helicobacter pylori infection is closely related to chronic gastritis, peptic ulcer and gastric cancer, which has been generally recognized by medical personnels.With the continuous progress of the researches, it was found that Helicobacter pylori infection is not only associated with diseases of the digestive system, but also may be related to oral diseases, blood system diseases, respiratory system diseases, circulatory system diseases and skin diseases, etc, playing a role in the process of their occurrence and development.Oral cavity is the initiation of the digestive tract; the anatomical structure has a certain similarity with gastrointestinal tract. Helicobacter pylori can plant in the mouth by fecal-oral route or esophageal reflux and live in mouth for a long time, so it is possible that Helicobacter pylori infection is associated with oral disease. The possibility has inspired researchers’interests, and they have achieved considerable results. Some studies have confirmed that Helicobacter pylori infection is associated with periodontitis,caries,recurrent oral ulcer,halitosis,oral lichen planus and dry socket. In the other hand, there are some studies present different views. So there still exist disputes about the relationship between Helicobacter pylori and oral diseases. Colloidal gold method is a new kind of detection technologies; it is more convenient, sensitive and safer, cheaper, comparing with the previous detection technologies. The researches using colloidal gold method to detect Helicobacter pylori in oral cavity have made a certain achievement, also proved that the colloidal gold method is a reliable detection method, and provided a reliable basis for this study. Saliva test cassette is produced based on the principle of colloidal gold method. It is easy to be used and be widely applied. Further more, it also can be applied in immediate detection and diagnosis. It may show its great superiority and play its role in a wider area in the future.ObjectiveTo study the correlation between Helicobacter pylori and common oral diseases, such as periodontal diseases, caries, and recurrent oral ulcer, by detecting Helicobacter pylori in saliva of check-up crowd by saliva test cassette.MethodsTotally1050saliva samples of adults were detected for Helicobacter pylori by saliva test cassette, including853cases of periodontal diseases,275cases of caries and10cases of recurrent oral ulcer.135healthy persons were selected as the control group, who received the same method to detect Helicobacter pylori. There are525male and525female,108people less than30years old,303people aged from30to39,269people aged from40to49,269people aged from50to59, and101people over sixty years old(including people who were sixty years old).190people with a history of gastric ulcer in contrast to860people without gastric ulcer. Then we compare the positive rate of Helicobacter pylori between different groups.Results1.The positive rate of Helicobacter pylori in male group and female group were59.62%and60.95%respectively, x2test suggested that the differences were not statistically significant(χ2=0.19, P>0.5). The positive rate was54.63%in people less than30years old;58.75%in people between30and39;67.29%between40and49;57.99%between50and59;58.42%over60years old (including people who were sixty years old). χ2test suggested that the differences were not statistically significant(%2=7.99, v=4, P>0.05).2. The positive rate of Helicobacter pylori in group with history of gastric ulcer and in group without history of gastric ulcer were69.47%and58.26%respectively, x2test suggested that the differences were statistically significant(χ2=8.18, P<0.05).3. The positive rate of Helicobacter pylori in control group was54.07%. The positive rate in group of caries, periodontal disease and recurrent oral ulcer were66.91%,63.42%and50%respectively. χ2test suggested that the differences were statistically significant except for recurrent oral ulcer.Conclusions1. The positive rate of Helicobacter pylori in patients group were significantly higher than that in control group, suggested that oral cavity is an important planting area for Helicobacter pylori, and the infection of Helicobacter pylori in oral may be closely associated with the occurrence and development of periodontal diseases and caries.The differences of positive rate between groups divided by age and gender were not statistically significant, suggested that age and gender were not the influence factors of helicobacter pylori infection.2. Test results were in accord with previous studies have been reported, it suggested that saliva test cassette is not only a kind of simple method, but also is reliable.3. The differences of positive rate between groups with recurrent oral ulcer and control group were not statistically significant; it maybe suggested that the infection of Helicobacter pylori was of no effect on recurrent oral ulcer. On the other side, it may be the result of statistical bias because of the small sample size. Next we need to increase the number of samples, go further to determine the correlation between Helicobacter pylori and oral diseases. Local therapy combined with anti-Hp-treatment may be worthy for a further research.
