The Effect Of ICU Preoperative Video Education On Patients With Cardiac Valve Replacement Operation

Objective:A video will be produced for preoperative interviewing and education of patients with cardiac valve replacement. A batch of nurses will be trained for preoperative interviewing and education with patients in ICU. The video will be used in clinical trials to compare the differences between two kinds of preoperative education methods, which named video interview and oral interview, for patients with the cardiac valve replacement operation in ICU. Finally, the novel effective video interview method will be established to enhance the effect of preoperative interview and quality of nursing care.Methods:(1) In the process, related literatures were read, methods and content for interview and education from other countries were integrated. Then outline of video for preoperative interview was composed after discussion by our group. Ten experts from critical medical care specialty and twenty patients with cardiac valve disease in perioperative period were invited to find out needs and instructions for interview contents.(2) The standard content of video interview by nurses from ICU for patients with cardiac valve replacement were made out, after three rounds of consultation for fifteen experts from the first affiliated hospital of Zhejiang University in specialty of hospital management, critical care, nurse education and critical medicine, which are related with works of ICU.(3) The scripts of video clip were composed on the basis of standard content for video interview to patients with cardiac valve replacement. After discussion in our group, scripts were put into effect. The initial video clip were reviewed by medical staff, patients and their family members in our department. Then second version was edit according to their responses. After preliminary testing on twenty patients with cardiac valve disease, the video clip was edit again and the final version was completed.(4) Following the principle of voluntary, nurses for preoperative interview were selected from ICU. The nurses’ interview group were founded after training and assessment.(5) All102patients with cardiac valve replacement were randomly divided into experimental group which contained54patients and control group which contained48patients, with different interview method for preoperative health education. In the experimental group, video interview was used, while oral education was adopted in control group. The State-Trait Anxiety Inventory was investigated before interview. After interview, State Anxiety Inventory and Visits Content Knowledge Questionnaire were inquired. State Anxiety Inventory and Satisfaction Questionnaire were surveyed in both groups before leaving department.Results:(1) The content standards of preoperative video interview for education of patients with cardiac valve replacement were established, after integration and organization of comments and suggestions from15experts.(2) After several modifications and edits, clip of preoperative video interview was produced for patients with heart valve replacement, which had8minutes.(3) In clinical trials, video education was used in the experimental group, while oral education was adopted in control group. Compare the differences between two kinds of preoperative education methods. The results showed that no statistical significant between the two groups in the anxiety levels (P>0.05), and the content awareness of interview and satisfaction in experimental group were higher than those of control group (P<0.05).Conclusion:The video clip for preoperative interview was suited for ICU, after several rounds of consultation for expert and several inquiries for patients with cardiac valve disease and their families. In this study, we showed that anxiety levels of patients were not affected by different methods of interview. However, video interview had advantages in content awareness and satisfaction compared to oral interview.
